• This position reports to the Social Work Manager. In this role, you will evaluate and diagnose a diverse array of psychosocial problems impacting your patients and their families, assisting them in overcoming personal and environmental challenges attributable to mental or physical illness.
• You will adhere to the professional standards of practice as outlined in the National Association of Social Workers (NASW) Code of Ethics, ensuring all interactions and interventions are ethical and compassionate.
• Your day-to-day responsibilities will include conducting comprehensive bio-psychosocial evaluations to identify and eliminate barriers to treatment, thereby enhancing patients' health outcomes.
• Collaborate with medical and support staff to provide integrated care by sharing insights about patients’ psychosocial functioning, contributing to the development of comprehensive care plans aimed at improving treatment compliance and overall patient experience.
• Develop individualized treatment plans incorporating both individual and group therapy modalities, regularly analyzing treatment progress to maximize health outcomes.
• Maintain accurate documentation of all social work activities in patients’ medical records as per department standards while complying with relevant laws and regulations.
• Educate patients and their families about medical and social work recommendations designed to help restore optimum functioning, thereby enhancing their quality of life.
• Advocate for patients by assessing their needs and coordinating access to community resources, working towards self-determination and improving their social support network.
• Stay informed about community resources relevant to your patients, participating in ongoing professional development to enhance your expertise and skills.
• Adhere to all departmental policies and safety protocols, facilitating the maintenance of a safe, compliant, and therapeutic environment.

Our patient population predominantly includes individuals facing a spectrum of mental health challenges, including anxiety, depression, and other psychosocial issues. • A Master’s Degree in Social Work from an accredited institution, accompanied by a current license as a Licensed Clinical Social Worker (LCSW) from the State Board of Social Work Licensure.

• A minimum of one year of clinical experience, ideally in health or mental health settings relevant to our patient demographics.
• Familiarity with quality improvement practices and evidence-based treatment modalities is crucial in driving successful patient outcomes.

• Experience working with diverse populations, particularly those experiencing socio-economic barriers to mental health care.
• Previous involvement in community resource coordination, advocacy efforts, or collaborative care models is highly advantageous.
• Strong communication and interpersonal skills, allowing for effective interactions with patients and all members of the healthcare team.

We offer a competitive salary range of USD 85,000 – 100,000 annually, commensurate with your experience and qualifications.
