Role objective:
The Community Liaison’s main job is to build long-term, effective partnerships with referral partners in the healthcare and senior-care community. Through those relationships, you will help increase the number of new patients who choose The Bernard Health Institute to help seniors and their families with skilled nursing and senior care.
Success in this role comes from showing up in person, earning trust, following through, and creating partnerships with healthcare professionals and organizations that produce referrals. You will be expected to work toward clear referral partnership and new-patient KPIs once hired and trained.
What you will do:
  • Build and nurture long-term relationships with referral partners such as social workers, discharge planners, senior-living staff, case managers, healthcare offices, and community partners 
  • Conduct in-person visits and follow-ups across Montgomery County to introduce The Bernard Health Institute and understand what each partner needs 
  • Represent Bernard at chambers of commerce, networking groups, community events, and related gatherings as assigned 
  • Deliver a clear, professional value proposition and leave partners with a strong reason to refer 
  • Keep an accurate outreach tracker (contacts, conversations, next steps, and outcomes) 
  • Follow up consistently so warm relationships do not go cold 
  • Participate in training, role-play, and occasional field shadowing to sharpen partner conversations 
  • Work toward agreed referral-partnership and new-patient KPIs set during onboarding and ongoing management
What this role is not:

  • Not a clinical care or caregiving position 
  • Not responsible for final intake or clinical care decisions 
  • Not a remote-only / social-media-only marketing job
Hours, pay, and schedule:
  • Part-time: 10 hours per week
  • Pay: $27+/hour plus a competitive bonus structure tied to results 
  • Schedule may include mornings, evenings, or weekends depending on partner availability and events 
  • Primary work area: Montgomery County, Maryland

Must-haves:

  • Valid driver’s license
  • Reliable car and ability to travel to in-person meetings across Montgomery County, MD
  • Smartphone for calls, texts, scheduling, and field follow-up 
  • Exceptional spoken and written English
  • Comfort walking into professional offices, chambers, and community settings (by appointment or appropriately unannounced) 
  • Reliable self-management in a field role — tracking contacts, keeping follow-ups, and working without constant supervision 
  • Preference for living in or near Montgomery County
  • Willingness to pass a background check if required
Optional but preferred:
  • CRM or outreach-tracker experience 
  • Healthcare, senior services, or home-care background 
  • Sales, marketing, or networking experience 
  • Bilingual
