Are you passionate about connecting people to the resources they need to thrive? The Delaware Division of Medicaid and Medical Assistance (DMMA) is looking for a compassionate, organized Health and Human Services Case Manager I to join its Newark team. In this entry-level case management role, you'll serve as a trusted advocate — helping individuals and families access the health, social, and community-based services that support their independence and well-being.
What You'll Do
  • Interview clients and determine eligibility for health and human services and benefits.
  • Conduct comprehensive assessments to identify clients' needs and develop personalized service plans.
  • Coordinate with community partners and service providers to facilitate access to essential services.
  • Maintain detailed case records and documentation to ensure efficient, effective service delivery.
  • Advocate on behalf of clients to ensure their needs are met and their rights are protected.
  • Provide emotional support and guidance, empowering clients to achieve their goals.
  • Participate in team meetings and collaborate with colleagues to identify and implement process improvements.
What We're Looking For
