You will have the support of a collaborative team of healthcare professionals, including psychiatrists, primary care physicians, and other specialists, ensuring a multidisciplinary approach to treatment. • Deliver psychotherapy and other mental health services to patients grappling with conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD).
• Conduct comprehensive assessments to evaluate patients' mental health, emotional states, and social functioning to develop individualized treatment plans.
• Design and implement evidence-based treatment strategies tailored to meet the unique needs of each patient, while integrating cultural competence into your approach.
• Monitor patient progress closely, adjusting treatment plans as needed based on observations and feedback from patients and multidisciplinary team members.
• Educate and support patients and their families by providing vital information about mental health conditions, available treatment options, and community resources.
• Uphold ethical standards and professionalism, ensuring the confidentiality of patients and obtaining informed consent to foster a trusting therapeutic relationship.

Your primary focus will be on patients with varying mental health challenges. You will work collaboratively with individuals undergoing significant life changes and families navigating complex care decisions, particularly in the hospice setting. This role demands not only clinical acumen but also empathy and flexibility to adapt to the emotional needs of patients and their loved ones.

• A Master's degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• At least two years of direct experience providing psychotherapy and mental health services.
• Exceptional communication and interpersonal skills are essential for effectively connecting with patients and collaborating with a variety of healthcare professionals.
• Sound clinical skills including the capacity to perform thorough mental assessments and create effective treatment plans.
• Proficiency in using electronic health records (EHRs) and other healthcare management software is crucial for coordinating care and maintaining accurate patient documentation.
• Knowledge and understanding of mental health disorders and treatment methodologies, alongside the ability to provide culturally sensitive care.

Preferred candidates will have experience working in hospice or palliative care settings, demonstrating an ability to support patients and families during challenging times. Familiarity with community resources and support services in the Rochester area will also be advantageous.
