Position: PACT Case Manager

Location: Aspen

A Cost-of-Living Adjustment (COLA) applies to onsite positions only based on applicable FTE. Telehealth employees are not eligible.

  • Hourly employees working in Aspenย receive a COLA of $3.88 per hour.

Position qualifications: ย ย Bachelorโ€™s degree required; preferably in human services or related field.ย  Bachelorโ€™s degree in a non-related field may perform the functions of this position only if they have one year experience in a behavioral healthcare field.ย  Previous experience with substance abuse and mental health-related case management and treatment preferred.ย  Minimum of two (2) years human service related experience preferred. Current valid Colorado Driverโ€™s License required. Certified Addiction Specialist is strongly preferred.

Job purpose: ย The purpose of the PACT (Pitkin Area Co-Responder Teams) program is to support law enforcement on calls for service involving mental health and/or substance use and, when possible, divert individuals with behavioral health disorders or symptoms from involvement in the criminal justice system, engaging them in treatment and/or social services instead.

The PACT case manager will primarily work with a diverse population including individuals who are struggling with mental health issues, substance use, and/or are involved in the criminal justice system. Patient population could include children, adults, individuals, couples, and groups. The case manager connects individuals and families to a wide range of resources and coordinates therapeutic and community-based services. The case manager may also assist with transportation as needed.

Knowledge, Skill & Ability (include materials and equipment directly used):

  • CPR/BLS Certification for Healthcare Provider prior to independent patient care required.
  • Working knowledge of Zoom, Teams and/or other tele-health platforms and applications.
  • Ability to produce written documentation of clinical work in a manner readily understood by other service providers.ย 
  • Ability to summarize service themes from a variety of client contacts and communicate suggested improvements for service delivery.
  • Ability to demonstrate appropriate clinical judgment when assessing the level of risk and determining resolution of client emergencies, resulting in a safe outcome.
  • Working knowledge of Windows based computer applications to include Microsoft Word, Microsoft Excel, Microsoft Outlook, and Internet access.
  • Ability to operate standard office equipment including computer keyboard, calculator, copy machine, fax and multi-line telephone.
  • Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
  • Ability to organize and prioritize workload in a sometimes-hectic environment with frequent interruptions.

Working Conditions/Physical Demands:

Employee must occasionally lift and/or move up to 25 pounds.ย  Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop.ย  Must have ability to sit for longer periods of time at a computer.ย  Employee must be able to travel between Health Solutions West sites.ย  May be exposed to stressful situations that involve enraged, distraught and/or intoxicated individuals with possible mental health conditions. Possible exposure to communicable disease.ย  The noise level in the work environment is usually moderate.ย  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
