Position Summary

As a COOR2 representative you will...

* Identify and triage various patient information from referral sources, the COOR2 fax triage, a specific mailbox in outlook or several different portals to the appropriate teams within the ENC for accurate patient follow up. The information will be used to either create a new account or upload the needed information to an existing patient account.

* Assess patient needs and document referral requests for coordination of care.

* Provide information pertaining to enteral nutrition services to applicable departments in order to bring a patient on service with Coram.

* Coordinate with the ENC Operations Support Team (Dietitian Team) to determine which items are on backorder and appropriate alternatives. Contact the patient directly to discuss details on backordered items, alternative items, and ship the orders to the patients.

*You must be able to work in a fast-paced environment and know when to pivot to accomplish tasks.

*You must be able to use your customer service training to be empathetic, patient, and communicate clearly and concisely both written and verbally.

* Training will occur virtually

* Position is fully remote -quiet working station, and high speed internet access is required

This is a full-time, permanent, benefited position and is NOT through an agency.


Required Qualifications

* Minimum 1 year experience working in a customer service or call center environment.
* Data entry experience.
* Working knowledge in Microsoft Office, specifically Outlook and Word.

The typical pay range for this role is:

$17.00 - $28.46

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
