Minimum Qualifications:
  • Possess a Bachelor's degree from an accredited college or university with a major in a human services field, in accordance with Medicaid Provider Manual Guidelines.
  • Experience with job coaching, supported employment, and/or Individual Placement and Support (IPS) with individuals with mental health conditions preferred.

Is helpful to others in the agency in getting work done, setting a tone of cooperation, models caring and cooperation behavior, shares resources and volunteers to help others succeed.
Provides coaching to team staff to build competencies that are necessary to effectively provide supported employment services including strength based interactions, person centered planning, Stage of Change, Motivational Interviewing, Co-occurring principles and interventions and practice protocols.
Assesses and evaluates the vocational needs of adult clients with severe and persistent mental illness and continues caseload contact to develop goals.
Assists clients in developing appropriate program plans to meet identified needs. Completes referral forms, clinical and legal documentation necessary to obtain community based services for clients. Maintains a record of client contact for agency documentation, team presentations, reviews and staff meetings.
Provides vocational case management services for support, rehabilitation and/or crisis intervention purposes. Monitors and evaluates vocational situations and settings to determine consistency of treatment and progress. Coordinates with the clinical team to monitor that medications are being taken, transportation needs are met and other basic needs are provided for.
