Job Summary:

The HUB Services Care Coordinator, LPN serves as the single point of contact for patients, caregivers, and/or physicians. The Care Coordinator, LPN receives inbound calls and provides education and support to individuals regarding assigned therapeutic areas, make outbound calls to enrolled patients to explain program services available to patients who opt into the services, and to record data necessary to patient support. The primary measures of the Care Coordinator, LPN performance are the delivery of results versus key operational metrics and client satisfaction. The below is intended to describe the general content of and requirements for the performance of this job and is not to be construed as an exhaustive statement of essential functions, responsibilities, or requirements.

Primary Duties and Responsibilities

  1. Receive and make calls to patients and/or caregivers and physicians regarding assigned disease states.
  2. Counsel and educate new/existing patients on the use of products, and therapy.
  3. Follow approved FAQs provided by the client.
  4. Support patients through education and setting of proper expectations of treatment and potential side effects.
  5. Contact the Physician’s office regarding questions, concerns, or problems with referrals, if applicable.
  6. Document all calls to meet requirements of management, regulatory compliance, and program specific procedures.
  7. Refer all reported adverse effects to the program RN for review and reporting following approved Standard Operating Procedures. In some cases, may be responsible for reporting AE’s directly to the entity used by the manufacturer.
  8. Participate in program specific customer meetings and training sessions.
  9. Participate in program specific orientation meetings and can demonstrate clinical competency on written tests.
  10. Work on special projects as required.
  11. Regular and reliable attendance expected.
  12. Other work-related duties as assigned by supervisor/manager.

Minimum Knowledge, Abilities, and Skills Required

  1. Minimum High School Diploma or equivalent.
  2. Must have a minimum of 2 years of nursing experience with at least 2 years of other clinical experience for a total of 4 years of nursing experience desired.
  3. Maintain current licensure as Licensed Practical Nurse in the state of Tennessee.
  4. Must be willing and able to work any Anovo-operating shift between the hours of 7am-7pm Monday-Friday.
  5. Willing to obtain additional licenses in states required for Call Center Nurses.
  6. Must have good computer and typing skills, proficient at Microsoft Office Suite, primarily Word, PowerPoint and Excel.
  7. Able to learn and effectively utilize HUB Software.
  8. Must not have history evidencing any dishonesty, lack of trustworthiness, or workplace violence.
  9. Willing to work the assigned shift to cover the hours of operation of the HUB Program.
  10. Attention to detail and committed follow through in communication with patients and providers.
  11. Empathic listening skills to interact effectively with patients and providers.

  13. Willing to work in fast paced environment and able to multi-task.
