Job description

Empath Home Health, a member of Empath Health is seeking a caring and motivated Speech Language Pathologist (SLP) - PRN to work with in Hernando County

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.
Location: home health visits in Hernando County (including Spring Hill, Weeki Wachee, Brooksville, and surrounding area)
Schedule: PRN; 5-10+ home health visits per week
What you'll Do
  • Provide skilled speech therapy services in accordance with physician orders, the plan of care, and regulatory standards
  • Complete initial and ongoing patient assessments, including required OASIS assessments
  • Develop individualized plans of care in collaboration with physicians and the interdisciplinary team
  • Educate patients and families on swallowing safety, dietary consistency, and communication strategies
  • Support cognitive, sensory-motor, language, and memory development through individualized therapeutic interventions

What You'll Need
  • Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
  • Active Florida SLP license
  • Minimum of 1 year of experience as a licensed Speech Language Pathologist (SLP)
  • Home health care experience preferred
  • Strong clinical assessment, communication, and patient-education skills
