This role is designed for flexibility, allowing you to work remotely from the comfort of your own home while having a significant impact on patients located in Atlanta, Georgia. You will interact with a multidisciplinary team consisting of healthcare professionals, including nurses and physicians, all dedicated to providing holistic care in a hospice center environment. As an LCSW at Harmony Family Support, your responsibilities will include:

• Conducting thorough clinical assessments to identify the mental health needs of patients and their families, formulating comprehensive treatment plans that follow evidence-based practices.

• Implementing therapeutic interventions, utilizing modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) tailored to each client's specific circumstances and cultural context.

• Maintaining clear and accurate clinical records in compliance with HIPAA regulations to ensure patient confidentiality and adhere to our quality standards.

• Collaborating with a dynamic team of healthcare professionals, participating in multidisciplinary team meetings to coordinate care and optimize patient outcomes.

• Engaging in regular case consultations and peer reviews, supporting your professional development and continuously enhancing care practices.

• Providing education and emotional support to patients and their families, guiding them through their treatment options, available resources, and the emotional processes of serious illness.

• Staying informed on the latest developments in mental health care, integrating new knowledge into your practice to improve patient treatment plans and outcomes.

This position focuses on patients experiencing a wide range of emotional and psychological challenges related to serious, chronic illnesses and end-of-life transitions. You will work closely with adults and senior patients, often addressing complex family dynamics, grief counseling, and mental health crises. Your role will ensure that patients and their families feel supported during a profoundly challenging time in their lives.

• A Master's degree in Social Work from an accredited institution is required to fulfill licensing requirements.

• A valid and unrestricted LCSW license to practice in Georgia is a must, signaling your commitment to maintaining high professional standards.

• A minimum of three years of clinical experience in a relevant setting, demonstrating your capability to provide psychotherapy and support services effectively.

• Strong knowledge of solution-focused psychotherapy interventions, highlighting your expertise in aiding clients towards positive change.

The annual salary for this full-time position ranges from USD 80,000 to 100,000, commensurate with experience and education.
