• Conduct thorough assessments to understand each patient’s mental health needs, personal circumstances, and develop appropriate treatment plans.
• Provide counseling and emotional support to individuals and families, integrating education about mental health and community resources available to them.
• Utilize electronic medical records (EMR) to document patient interactions, track progress, and communicate effectively with your interdisciplinary team.
• Ensure compliance with legal mandates regarding reporting vulnerabilities or suspected abuse when necessary.
• Engage proactively with patients who are at risk for readmission by recognizing signs and utilizing technology for early intervention.
• Plan and facilitate transition conferences for patients and their families, ensuring a comprehensive understanding of care plans.
• Collaborate with healthcare providers and community resources to create an interconnected support system for patients.
• Advocate for patient rights, ensuring ethical decision-making processes and access to necessary protective services.

Your role as a Licensed Mental Health Counselor will primarily involve working with a diverse population of patients dealing with a range of mental health challenges, including but not limited to anxiety, depression, stress management, and coping with chronic or terminal illnesses. You will also provide support to families and caregivers, identifying their needs and implementing strategies to help them cope effectively.

• A Master’s or Doctoral degree in Clinical Psychology, Counseling, or Clinical Social Work from an accredited program (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).
• An active, independent license (LCSW, LMFT, LPC) within the state in which you reside.
• Demonstrated ability to manage a substantial caseload and effectively communicate with an interdisciplinary team and support staff.
• Proficient application of electronic medical records in managing patient care documentation.
• Strong verbal and written communication skills.

• Experience working with patients in a community health setting is highly desirable.
• A minimum of two years in a similar mental health counseling role is preferred.
• Familiarity with the latest mental health therapies and methodologies.
