• Conduct thorough assessments and screenings by utilizing telehealth platforms to explore clients' mental health challenges and develop personalized treatment plans that align with their individual goals.

• Facilitate evidence-based individual and group therapy sessions, tailoring therapeutic approaches to the unique emotional needs of clients residing in hospice care.

• Collaborate closely with an interdisciplinary team that includes physicians, nurse practitioners, social workers, and case managers to create a seamless care experience for clients and their families.

• Provide psychoeducation to families about mental health processes and how they can support their loved ones throughout the therapeutic and recovery journey.

• Maintain meticulous and confidential records in accordance with regulatory requirements and ethical standards, ensuring compliance following applicable mental health practices.

The primary focus of our mental health therapy services is on patients within the hospice environment experiencing emotional distress. • A current and valid license as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).

• A bachelor's degree in social work, psychology, counseling, or a closely related field.

• At least two years of demonstrated experience in mental health counseling, with a preference for those experienced in hospice or palliative care settings.

• Strong foundational knowledge of evidence-based mental health practices tailored to various disorders and patient needs.

• Exceptional communication skills, both verbal and written, enabling effective rapport-building with clients and collaborative teamwork.

• A commitment to ethical practices and maintaining client confidentiality in all therapeutic processes.

• Specialized training or certification highlighting advanced competencies in grief and loss counseling or trauma-informed care approaches.

• Experience working within interdisciplinary teams to provide holistic patient care.
