About the Job:
We're looking for a Family Nurse Practitioner to join our collaborative team. You'll focus on early intervention, chronic disease management, and long-term wellness in a setting that values quality over volume.
What You'll Do (Key Responsibilities in Patient Care):
  • Comprehensive Care: Provide both in-person and virtual visits to increase access and continuity of care.
  • Diagnosis & Planning: Prescribe medications and develop personalized health plans with your patients.
  • Wellness Education: Educate patients on wellness, prevention, and lifestyle management.
  • Collaboration: Collaborate closely with physicians, medical assistants, and other clinicians.
  • Culture: Contribute to a positive, team-oriented clinic culture.

What You'll Bring to the Team (Minimum Qualifications):
  • Active and unrestricted FNP license in the practicing state.
  • Board certification as an FNP.
  • DEA registration.
  • 2+ years of primary care experience preferred.
  • Experience treating patients ages 2+.
  • Familiarity with HIPAA and a strong commitment to patient confidentiality.
  • Ability to pass on-site credentialing.
