Job Title: Care ManagerJob Description

The Care Manager provides comprehensive case management and care coordination services for elderly individuals and adults with physical and intellectual disabilities, helping them live as independently as possible in the community. This role combines social work case management, member assessments, and collaborative care planning within a mission-driven, team-oriented environment. The position is full-time, primarily weekday-based, and supports a partnership program serving members in Milwaukee and the surrounding area.

Responsibilities
  • Manage an assigned caseload of program members in collaboration with an RN Care Manager, ensuring timely and effective delivery of services.
  • Complete comprehensive member assessments, including physical, social, and psychosocial evaluations, to determine needs and appropriate services.
  • Develop and maintain member-centered care plans that promote independence and support individual goals and preferences.
  • Perform and document functional screens as required, including certified functional screener responsibilities when applicable.
  • Provide ongoing care coordination, ensuring services are in place, appropriate, and adjusted as member needs change.
  • Monitor the quality of services provided to members and take action to address gaps, concerns, or opportunities for improvement.
  • Maintain all required documentation and member-related paperwork in adherence to established standards, policies, and regulatory requirements.
  • Provide direct social work case management services, including advocacy, resource linkage, and psychosocial support for members and their families.
  • Facilitate team-based case management, collaborating closely with interdisciplinary team members such as nurses, social workers, and other care professionals.
  • Work effectively within healthcare management systems and managed care environments, including programs such as Medicare, Medicaid, and BadgerCare.
  • Coordinate care and services for members within the broader healthcare industry, ensuring integration of medical, social, and community resources.
  • Engage in regular communication with members, families, and providers to support continuity of care and address emerging needs or concerns.
  • Participate in team meetings and case conferences, contributing professional input to care planning and service coordination.
  • Use personal vehicle for regular local travel within Milwaukee County to conduct home visits, community visits, and other member-related activities.
  • Adhere to all organizational policies, ethical standards, and professional social work guidelines in daily practice.
Essential Skills
  • Master’s degree in Social Work required; the master’s degree must be in Social Work and no other master’s degree will be considered.
  • Current Wisconsin Social Work licensure required.
  • Minimum of 1 year experience providing care or services for a frail or elderly population required.
  • Knowledge of care management and care coordination practices for elderly individuals and adults with physical and intellectual disabilities.
  • Ability to complete comprehensive member assessments and develop member-centered care plans.
  • Strong skills in social work case management, including advocacy, resource coordination, and psychosocial support.
  • Proficiency in maintaining accurate, timely documentation in adherence to established standards and regulatory requirements.
  • Understanding of Medicare, Medicaid, BadgerCare, and managed care programs within the healthcare industry.
  • Familiarity with healthcare management systems and processes related to care management and social services.

Additional Skills & Qualifications
  • Bachelor’s degree in Human Services or a closely related area may be considered as an alternative pathway.
  • A minimum of three (3) years of experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services bachelor’s degree.
  • One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity is preferred.
  • 1 to 2 years of case management experience with elderly and/or developmentally or physically disabled populations is preferred.
  • Background in social work, psychology, or human services is beneficial.
  • Certified functional screener experience or eligibility is a plus.
  • Experience working within healthcare management, care management, or care coordination settings is advantageous.
  • Comfort using healthcare management systems and electronic documentation tools.
  • Interest in working in a mission-driven organization focused on supporting independence for elderly and disabled adults.
  • Ability to thrive in a collaborative, team-oriented environment with community-based case management responsibilities.
Work Environment

The role is based in an office setting in Milwaukee County, with initial training conducted at the Vliet location. Standard hours are Monday through Friday, 8:00 AM to 4:00 PM, providing a consistent weekday schedule. Telework is currently common, with flexibility to work remotely as appropriate, while still requiring regular local travel within Milwaukee County to visit members in the community. The position involves working within healthcare management systems and managed care programs, and requires the use of a personal vehicle, a valid driver’s license, and appropriate insurance coverage. Pay and Benefits

The pay range for this position is $23.00 - $35.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
