Value Based Care LPN for DMC at Home Program
DMC Primary Care seeks an experienced Licensed Practical Nurseย (LPN) to join our DMC At-Home Program as a Value Based Care/At-Home Program Nurse (LPN). The DMC At-Home team provides wrap-around care to a population of DMC patients who are not able to visit our offices in person.
As a Value Based Care Nurse, you will serve as a key member of the At-Home Program care team, supporting the Advanced Practice Providers (APPs) who deliver in-home medical services through telephonic care coordination and patient management.
You will play an integral role in transitions of care by contacting patients following hospital discharge, reviewing discharge instructions, performing medication reconciliation, updating electronic medical records, and facilitating communication between patients, providers, caregivers, and care management teams.

Through proactive care management and chronic disease support, the Value-Based Care Nurse helps patients navigate their healthcare needs, promotes adherence to treatment plans, and addresses barriers to care. The overarching goal of this role is to improve health outcomes and quality of life for patients with chronic and complex conditions, reduce avoidable emergency department visits and hospitalizations, and support value-based care initiatives that promote high quality, patient-centered care.

The successful candidate for this position will possess high-level organizational skills and the ability to multi-task and work under pressure, while presenting a calm, professional manner and positive attitude.
This is a full-time position, Monday through Friday, located in our DMC At-Home office in Bedford, NH.
