Position Summary - The Cumberland Community Health Worker Program Facilitator serves as a bridge between healthcare providers and the community, promoting health education and access to services. They will engage with individuals and families to assess health needs, provide information as needed. They will facilitate communication between patients and healthcare systems, helping to navigate resources and address barriers to care. They will conduct outreach activities, coordinate health screenings, and advocate for community health initiatives.

The Program Facilitator delivers community-based physical activity programming for adults age 50 and older as part of a rural health grant initiative. This position leads structured group sessions, encourages sustained participation, and supports participants in building safe and consistent physical activity habits, in accordance with the established program protocol.

The ideal candidate will be a student currently pursuing an associate or bachelor's degree in physical therapy, exercise science, kinesiology, health sciences, sports medicine, or a related field. The position provides an opportunity to apply academic knowledge in a community setting while gaining experience in group instruction, participant engagement, behavior change support, and community wellness programming.

The Program Facilitator will lead and demonstrate group physical activity sessions, provide encouragement and general physical activity education within the scope of their training and the program protocol, and escalate participant questions or concerns to the Principal Investigator/Clinical Director. Facilitators do not perform health screenings, clinical assessments, or clinical evaluations of any kind.

This is a part-time, temporary, grant-funded position with a current project end date of October 30, 2026. The position may be extended beyond this date, based on continued grant funding and business needs.

All facilitators must complete required training on program content and protocol prior to the start of the program. This is an onsite position that provides services at multiple community locations throughout Cumberland County, New Jersey. The primary work location will vary based on program and operational needs. Regular travel between community sites is required. This position is not based at the organization's home office, but rather at multiple sites within Cumberland County.

Essential Functions -

• Support community outreach and engagement activities as part of the rural health grant program.
• Lead and facilitate group physical activity sessions for adults age 50 and older in accordance with the established program protocol, training, and scope of practice.
• Prepare and organize session materials, supplies, and participant sign-in information for scheduled program sessions.
• Conduct group physical activity sessions appropriate to the individual's education, training, and scope of practice.
• Provide participants with general physical activity and healthy lifestyle education within the scope of their training and the program protocol.
• Demonstrate and facilitate bodyweight exercises, stretching, mobility, and other physical activity as directed by the program protocol.
• Encourage and motivate participants to engage in healthy physical activity and wellness practices.
• Communicate participant questions, concerns, or observed issues to the Principal Investigator/Clinical Director for follow-up.
• Provide information about available healthcare, wellness, and community resources as directed.
• Maintain accurate and timely documentation of session attendance, participant encounters, and program activities in the designated system.
• Utilize a tablet or other company-provided technology to document services and facilitate program activities.
• Maintain participant confidentiality and comply with HIPAA, organizational policies, grant requirements, and applicable procedures.
• Follow established safety protocols and program procedures during community-based sessions.
• Work collaboratively with the Project Manager and project team to support program goals and maximize productivity.
• Complete all required training on program content and protocol prior to the start of the program, and participate in ongoing training as directed.
• Attend the weekly group program huddle, held remotely by digital meeting.
• Travel regularly between community locations throughout Cumberland County.
• Perform other duties as assigned in support of program and grant objectives.

Prerequisite Qualifications -

1. Education/Degree Required

High school diploma or equivalent required.

Currently enrolled in an associate or bachelor's degree program in Physical Therapy, Exercise Science, Kinesiology, Health Sciences, Sports Medicine, Public Health, or a related field preferred.

2. Years of Experience Required

Entry-level position; prior professional experience is not required.

Experience participating in community health, fitness, wellness, or outreach activities is preferred.

3. Other Requirements

Valid driver's license and reliable transportation.

Ability to travel regularly between multiple community locations.

Ability to work a flexible schedule, including occasional evenings or weekends, as required by community events and program needs, and to attend a weekly remote program huddle.

Ability to safely demonstrate and participate in basic physical activity and exercise activities.

Ability to follow the established program protocol and recognize when a participant question or concern should be escalated to the Principal Investigator/Clinical Director.

Must be comfortable working directly with individuals from diverse backgrounds and communities.

Preferred Qualifications -

• Current enrollment in a Physical Therapy, Exercise Science, Kinesiology, Health Sciences, Sports Medicine, Public Health, or related degree program.
• Coursework or training in anatomy, physiology, exercise physiology, physical activity, wellness, or related areas.
• Prior experience leading group exercise or fitness sessions, wellness programs, community outreach, or health education.
• Bilingual skills are preferred to effectively communicate with the diverse communities served.
• Strong verbal, written, and interpersonal communication skills.
• Positive, encouraging, and approachable personality.
• Strong interest in community health, preventive health, fitness, and wellness.
• Ability to engage participants and establish rapport in a community-based environment.
• Keen attention to detail and ability to accurately document participant information.
• Ability to recognize potential concerns and appropriately escalate issues to the Principal Investigator/Clinical Director.
• Strong organizational skills and ability to prioritize tasks.
• Basic computer skills, including familiarity with Microsoft Office Suite and electronic documentation systems.
• Comfortable learning and using tablets and other technology used to support community-based programming.
• Collaborative working style and team-oriented approach.

• Ability to work independently while maintaining communication with the project team.
• Ability to adapt to changing community locations, schedules, and program needs.

Salary Information-In compliance with the NJ Pay Transparency law, the hourly rate for this position is $25.00. NJII considers factors like, but not limited to, job scope, experience, education, skills, internal equity, and market conditions when making offers. This pay range covers base pay only and excludes bonuses or other incentives.

Benefits: This is a part-time position and is not eligible for the company's employee benefit programs. Employees are eligible to accrue New Jersey Earned Sick Leave in accordance with applicable law, up to 40 hours per benefit year.

Travel: This position requires regular travel throughout Cumberland County, NJ. Business-related travel expenses will be reimbursed in accordance with Company policy and applicable grant guidelines.
