Description
Case Worker needed for full-time, temporary role in Montgomery County.
The Case Worker coordinates inpatient and post-acute services, including assessments, treatment planning, discharge planning, and aftercare coordination. This role collaborates with patients, families, physicians, and community providers to identify needs, secure resources, and develop effective discharge plans from admission through transition to the next level of care. Responsibilities include documenting all discharge-related activities in the TIER system, facilitating referrals, and providing guidance and support to help patients and families navigate the impact of illness and plan for successful recovery.
Position Requirements
BSW and at least two years of case management experience, preferably in discharge planning; an MSW is preferred. Strong knowledge of community resources, healthcare systems, and care coordination is required, along with excellent communication, documentation, organizational, and problem-solving skills. The ability to manage a high-volume caseload, work independently, and collaborate effectively within a multidisciplinary team is essential.
