Windward Home Health is seeking a Director of Patient Care Services (30 hours per week) to join our team! We are honored to be a San Diego Union-Tribune Top Workplaces award winner for seven years running, 2019-2025.
Proudly serving San Diego County for 22 years, Windward Life Care and Home Health is a group of trusted Aging Life Care ® and in-home care professionals with a premier reputation in our field. Windward Home Health offers RN/LVN services to clients who are billed on a private-pay basis. Our mission is to improve the lives of older and disabled adults in San Diego County by fostering a lifelong sense of purpose and maximizing independence.
The Director of Patient Care Services is a Registered Nurse (RN) who oversees all clinical services. This role is responsible for ensuring that care is delivered effectively, compassionately, and efficiently through managing the coordination of services and providing support and expertise to our team of home health nurses.
Responsibilities include:
Clinical Oversight:
  • Manage a team of RNs and LVNs (currently, 6) providing comprehensive in-home patient care
  • Oversee company clinical services, arranging for required 24 hours/day professional access. This includes cost accountability, service quality achievement, and assigned employee productivity (may delegate duties, as needed)
  • Manage development of employee qualifications and the assignment of employees and coordination of patient care services, coordinating care with other programs as needed
  • Oversee, participate in, and support the company Performance Improvement Committee activities establishing measurable outcomes/objectives. Assist in data collection to complete annual reporting requirements
  • Participate in budget, strategic plan, clinical policy and procedure development and implementation
  • Perform or delegate orientation, competency testing, and in-services for new employees; select ongoing quarterly education for RNs and LVNs
  • Follow up on patient and employee concerns as needed

Compliance:
  • Oversee the Windward Home Health Infection Control program; advise on infection control processes and best practices for Windward Life Care (non-medical home care)
  • Maintain confidentiality of patient, employee and company operations
  • Keep all records up to date, organized, and ready for inspection at any time
  • Maintain in-depth knowledge of and abide by all company policies and procedures; maintain current knowledge of home health care regulations and standards

Leadership:
  • Support the company mission statement and philosophy to promote quality, efficient, comprehensive, compassionate, and effective patient care
  • Supervise and serve as a role model and consultant to company employees. Is available on premise or by telecommunications during scheduled hours, devoting sufficient hours to assure quality and adequacy of services provided
  • Serve as Administrator's Designee, as requested

Work Schedule: 8:30AM - 3:00PM, Monday through Friday. (There is some flexibility in the start and end times, so if this work schedule doesn't exactly line up with your preferred work schedule, please don't let that stop you from applying!) This position will require work as needed in client residences throughout San Diego County, remotely, and from the company's office in Banker's Hill. This is a benefits-eligible position at approximately 30 hours per week.
Pay: $58.00 - $65.00 per hour
Requirements
  • Current California state license as a Registered Nurse (RN)
  • Bachelor of Science in Nursing (BSN)
  • Graduate of an accredited school of nursing
  • CPR certified
  • 3+ years clinical experience within the last 5 years, preferably in-home health agency, company, primary clinic or facility, at least 1 year of which was in a supervisory or administrative capacity. Some experience working with older and/or disabled adults preferred.
  • Familiar with quality, clinical, and financial reports
  • Written proof of health status and ability to lift, push and pull up to 40 lbs. unassisted
  • Knowledge of home health care regulations and procedures
  • Effective communication skills and ability to lead a team of employees with a variety of backgrounds and educational experiences
  • Excellent time management, problem-solving skills, and work ethic
  • Active driver's license, clean driving record, and reliable vehicle - must be comfortable driving patients in your vehicle
  • Moderate+ technology proficiency, including use of cell phone apps and web-based office systems, such as Google Workspace
  • Ability to pass criminal and driving background checks
