Seeking Family Nurse Practitioner or Physician Assistant - Mercy Health Glandorf, OH Family Medicine
Mercy Health - St. Rita's in Lima, Ohio is seeking a family nurse practitioner or physician assistant to provide primary care at the Mercy Health Glandorf, OH Family Medicine practice.
  • Primary Care
    • All age groups - pediatric to geriatric
  • Assess/evaluate, complete patient histories, diagnose, establish plan of care for patients.
  • Provide information, education, and support to patients/families regarding disease process and treatment plans. Facilitate strong communication between patients and the healthcare team.
  • Order medications, treatments, labs, etc. as described and permitted under the Ohio State Medical Board and the Mercy Health St. Rita's credentialing boards.
  • Counsel and advise patients regarding concerns.
    • Request consultations and initiate referrals to other healthcare providers including specialists, as needed.
  • BLS required
  • Call will be required.
  • Respond to patient calls and MyChart messages.
  • Monday to Thursday - 8 AM to 4:30 PM, Friday 8 AM to 12 noon.

Education and Skills Required:
Current licensure with the state of Ohio as a Family Nurse Practitioner or a Certified Physician Assistant.
Good verbal and written communication skills.
