Job description

Adventist Rehabilitation - RockvilleIf you are a current Adventist HealthCare employee, please click this link to apply through your Workday account.Rockville Rehabilitation seeks to hire an experienced Care Navigator Supervisor for our inpatient rehab hospital who will embrace our mission to extend God's care through the ministry of physical, mental, and spiritual healing.
**Qualified candidates will be eligible for an $8,000 sign-on bonus.
As a Care Navigator Supervisor you will:
Leads team to ensure that delays to care are minimized and that effective discharge plans are developed to navigate patients to the most appropriate level of care in a timely manner
Supports departmental and organizational financial goals of minimizing potentially avoidable utilization and length of stay as well as reducing readmission rate
Completes initial discharge needs, psychosocial assessment, and review of clinical needs of patients with focus on ensuring patient's safe discharge.
Coordinates and ensures active participation in Interdisciplinary Rounds and other patient care conferences
Submits and follows up on Continuing Stay Reviews (CSRs) on patients with non-Medicare third party payors and manages insurance benefit during length of stay.
Leads delivery of care provided by Care Navigation team to ensure that patients' needs are addressed, and that documentation meets standards
Completes monthly rounding and mentoring with assigned staff to facilitate employee engagement and clinical and professional advancement
Coordinates all aspects of staffing, scheduling, and patient experience in collaboration with the Director of Care Navigation
Collaborates with the Director of Care Navigation to help facilitate the development of staff, systems, processes and PI that drives optimal performance for the department.
Identifies and navigates patient appointments and treatment to reduce barriers to patient discharge; communicates barriers to leadership for resolution and trending
Other duties as assigned by department leaders.
Qualifications include:
Bachelor of Science in nursing or social work required, Master's degree preferred
3 - 5 years' experience in patient navigation, discharge planning, or care coordination
At least one year of prior leadership experience preferred
Maryland RN licensure
Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance as directed by AHC
Active American Heart Association Basic Life Support (BLS) certification required

Work Schedule:

  • Employment Type: Full-time

  • Hours per Week: 40 hrs/week

  • Typical Daily Schedule: Monday through Friday

  • Shift Type: Day

  • Details: 70% clinical, 30% supervisors

$8,000 sign-on bonus over 3 installments (paid at 90 days, 1 year, & 2 years of hire date) *Current AHC employees are not eligible for a sign-on bonus who are transferring from one entity to another and/or transferring within their entity. Employees that have terminated their employment with AHC are not eligible for a sign-on bonus prior to six months of returning if hired for a position offering a sign-on bonus.

Pay Range:

$83,112.12 - $124,675.20

If the salary range is listed as $0 or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.
