Duke Health Integrated Practice comprises more than 110 primary and specialty outpatient clinics, extending the reach of the Duke Health mission across the state of North Carolina.

Job Summary
We are seeking an Advanced Practice Provider in the Duke Breast Risk Assessment Clinic located in Durham, NC to work in collaboration with the clinic director to continue to expand the program and provide expert-level care to patients with risk factors for breast cancer. The APP will work with attending physicians to provide direct patient care services, collect and document data, conduct diagnostic/therapeutic procedures, and order/schedule imaging studies and professional consultations.
Primary hours will be Monday through Friday, day shift, with flexible hours to meet the needs of the patients.
Job Duties & Responsibilities
- Provide direct patient care services for new and return patients, both in person and virtually, including triage, physical exams, collecting and documenting data, and coordinating care.
- Counsel and educate patients on breast cancer risk and management recommendations.
- Conduct diagnostic and therapeutic procedures (such as skin punch biopsies).
- Order and schedule imaging, laboratory studies, and professional consultations.
- Conduct pre-authorizations for recommended studies, as ordered by the APP and/or other team members.
- Prescribe appropriate interventions and medications.
- Follow-up test results and coordinate ongoing care, including but not limited to imaging and pathology results.
- Conduct timely e-communication consults from referring providers.
- Communicate verbally and in writing all pertinent patient information to collaborating physicians, referring providers, nursing staff, patient resource manager, and other relevant healthcare providers.
- Provide timely documentation in the electronic medical record of all relevant data including results of tests; laboratory results; patients' condition and response to therapies/interventions; communications with colleagues.
- Patient care responsibility will be completed in an out-patient clinic setting and will not involve in-patient or operating room time.
- Facilitate educational experiences for trainees (students, residents, fellows, etc) and orientation sessions for other team members.
- Seek out and participate in learning opportunities through reading, attending seminars/webinars/conferences, etc, to enhance knowledge in breast cancer risk.
- Serve as a role model and mentor for team members, demonstrating professionalism in conduct, speech, appearance, work habits, customer service, communication style, critical thinking skills, resource utilization, and problem resolution.
- In collaboration with the clinic director (with dedicated time):
- Serve as an ongoing resource and primary point of contact for patients and providers in the Breast Risk Assessment Clinic.
- Collaborate the DCI New Patient Coordinators, Breast Imaging, the Clinical Cancer Genetics Program, Breast Surgical Oncology, and Plastics and Reconstructive Surgery,
- Working with epic champions/informatics specialists, create and manage note and discussion templates for team distribution and use.
- Communicate with team regarding practice advisories, updates, expectations, policies, and procedures.
- Review clinic metrics regularly to help develop and coordinate quality improvement projects and best practice initiatives.
- Working with DCI and Surgery administration, help create and optimize clinic templates to facilitate patient care and utilize clinic space appropriately.
- Lead and facilitate team meetings to identify, prioritize, and discuss any issues relating to clinical/professional practice and the success of the interprofessional team.
Job Eligibility Requirements
Education
- Nurse Practitioners: Work requires completion of an accredited Nurse Practitioner program, with a master's degree in nursing.
- Physician Assistants: Work requires completion of an accredited Physician Assistant program. A bachelor's degree required; master's degree strongly preferred.
Certification
- BLS required.
- Additional training and/or certification favored, such as APP certification through ASBrS, completion of the City of Hope Intensive Course in Genomic Cancer Risk Assessment, and CGRA (Cancer Genetics and Risk Assessment) certification through NCoBC
License
- Nurse Practitioners: Must have current or compact RN licensure & NP approval to practice in the state of North Carolina. National certification as a NP required.
- Physician Assistants: Must be licensed in the state of NC and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants.
Experience
- 5+ years of APP experience highly desired.
- Experience in a high-risk breast clinic preferred.
- Experiences in a breast surgery clinic and/or hereditary cancer clinic favored.
