Director of Case Management
Greater Nashville, Tennessee

The Director of Case Management oversees all departmental activities, ensuring compliance with organizational policies and regulatory agencies while leading the integration of patient care and discharge planning processes with hospital departments and external agencies for continuity of care and optimal Resource Utilization. The Director manages daily operations, engages interdisciplinary teams and stakeholders, and is accountable for achieving clinical, service, and financial outcomes. The RN Director requires executive presence, excellent communication and presentation skills, proven motivational leadership, and a strong record of delivering results and meeting goals.

The Director serves as a subject matter expert in Case Management, demonstrating solid leadership, trust-building, team building, and Change Management abilities to drive departmental success and initiatives. The Director of Case Management demands a self-starter who can manage multiple priorities, work independently on projects, adapt quickly to change, and coordinate across stakeholders while exercising tact and diplomacy in conflict situations.

Qualifications:

  • Registered Nurse (RN) with acute hospital nursing experience required
  • Bachelor Degree required
  • Director of Case Management in an Acute Hospital Setting required
  • Bachelor's or Master's in Nursing (BSN/MSN) preferred
  • Certification in Case Management (CCM/ACM) preferred

Responsibilities:

  • Directs and evaluates departmental operations, including the Case Management model, staffing (skill mix and FTEs), information technologies, onboarding, and staff competencies to meet performance and quality objectives
  • Allocates resources to ensure effective staffing, productivity, and quality goals
  • Provides oversight of core Case Management functions and practices
  • Ensures adherence to Care Coordination and discharge planning processes, including timeliness, quality, and accurate documentation
  • Collaborates with the CFO and the Divisional Case Management Leader to achieve established goals and expectations
  • Assesses and improves departmental performance by evaluating processes, analyzing data, and implementing sustainable performance improvement activities
  • Ensures compliance with policies, procedures, and standard operating procedures (SOPs)
