We're looking for a Care Manager who shares that conviction and wants to make a real difference in the lives of our clients and the caregivers who serve them.
The Role
As Care Manager, you are the heart of the operation - the bridge between our clients, their families, and our caregiving team. You'll oversee the quality and continuity of care across our client base while mentoring, supporting, and supervising the caregivers who make our mission possible.
What You'll Do
  • Conduct in-home client assessments and build personalized care plans that reflect each client's needs, routines, and preferences
  • Supervise, coach, and support our caregiving team, ensuring every client receives consistent, concierge-level care
  • Match caregivers to clients thoughtfully, with attention to skills, personality, and continuity
  • Perform regular quality-assurance and supervisory visits in clients' homes
  • Serve as the primary point of contact for families, communicating with warmth, clarity, and responsiveness
  • Monitor changes in client condition and adjust care plans in coordination with families and healthcare providers
  • Support caregiver onboarding, ongoing training, and compliance documentation
  • Help maintain scheduling continuity and assist with on-call coordination as needed
