The Family Nurse Practitioner (FNP) provides comprehensive primary healthcare services to patients across the lifespan, including assessment, diagnosis, treatment, health promotion, disease prevention, and chronic disease management. The FNP collaborates with the interdisciplinary healthcare team to deliver high-quality, evidence-based, culturally respectful care while supporting the mission and goals of Wassaja Memorial Health Center.

KNOWLEDGE, SKILLS, AND ABILITIES:

• Knowledge of evidence-based clinical practice guidelines.
• Knowledge of AAAHC accreditation standards and quality improvement principles.
• Ability to independently assess, diagnose, and manage acute and chronic illnesses.
• Excellent clinical judgment and decision-making skills.
• Strong communication and patient education skills.
• Ability to work collaboratively within a multidisciplinary healthcare team.
• Proficiency in Electronic Health Records (EHR) and Microsoft Office applications.

MINIMUM QUALIFICATIONS:

Licensure

• Current unrestricted Arizona Registered Nurse license.
• Current Arizona Advanced Practice Registered Nurse (APRN) license.
• National certification as a Family Nurse Practitioner (FNP-C or FNP-BC).
• Current unrestricted DEA registration.
• Current Basic Life Support (BLS) certification.
• Current Arizona Driver's License and ability to meet FMYN insurance requirements.

Experience

• Minimum of two (2) years of experience as a Family Nurse Practitioner in primary care, family medicine, rural health, or community health preferred.

• Experience working in Tribal or Indian Health Service facilities preferred.
• Experience managing chronic diseases including diabetes, hypertension, and cardiovascular disease preferred.

ESSENTIAL FUNCTIONS: The following duties are representative of the position and are not intended to be all-inclusive.

Clinical Responsibilities

• Perform comprehensive and focused health assessments for patients of all ages.
• Diagnose and manage acute and chronic illnesses using evidence-based clinical guidelines.
• Order, interpret, and evaluate laboratory tests, diagnostic studies, and imaging.
• Prescribe medications and therapeutic interventions within Arizona scope of practice and DEA regulations.
• Develop individualized treatment plans in collaboration with patients and families.
• Perform minor office procedures within scope of practice.
• Provide preventive care services including wellness examinations, immunizations, health screenings, and chronic disease management.
• Monitor patient progress and adjust treatment plans as clinically indicated.
• Coordinate referrals to specialty providers and community resources.

Chronic Disease Management

• Manage patients with diabetes, hypertension, cardiovascular disease, obesity, asthma, COPD, and other chronic conditions.
• Promote evidence-based management of chronic diseases through patient education and follow-up.

• Review quality metrics and identify opportunities to improve patient outcomes.
• Support the Special Diabetes Program for Indians (SDPI) and other chronic disease initiatives.

Patient Education

• Educate patients and families regarding diagnoses, medications, treatment plans, disease prevention, nutrition, and healthy lifestyle choices.
• Encourage patient engagement and shared decision-making.
• Provide culturally appropriate education that respects Tribal traditions and values.

Quality Improvement & Accreditation

• Participate in Quality Improvement (QI) and Performance Improvement (PI) initiatives.
• Support AAAHC accreditation activities and compliance with accreditation standards.
• Assist with development and implementation of clinical policies, procedures, and evidence-based practice guidelines.
• Participate in diabetes audits, preventive care initiatives, and quality reporting.
• Attend provider meetings, medical staff meetings, and required committee meetings.

Administrative Responsibilities

• Complete timely and accurate documentation in the Electronic Health Record (EHR).
• Review laboratory results, imaging reports, and specialty consultations promptly.
• Complete medication refills, prior authorizations, referrals, and required medical documentation.
