Job description

We are seeking a full time Case Manager to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder in Stark County. The Case Manager will assist with accessing community based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services. The Case Manager will work with Probation, Parole, Courts, Social Security, DJFS and other providers. Must have BA in related field, one year related experience and knowledge of community resources.

Must have, or be willing to obtain, an NPI number and Medicaid Provider Number. Must have and maintain an excellentdriving record.
