Job description

Habilitation Manager – Oahu Habilitation Office Position Summary

In collaboration with the Clinical Manager, the Habilitation Manager is responsible for leading field staff in delivering excellent client service, quality, and adherence to policies and procedures. This role also serves as an internal case manager and advocate for clients in the Habilitation program.

Salary: starting at $47k with quarterly performance-based incentives eligibility

Key Responsibilities Client Care and Case Management
  • Participate in person-centered planning processes and collaborate with clients, families, case managers, Habilitation Technicians, and interdisciplinary team members to implement and evaluate client care needs and individual goals.

  • Perform admission intake, when applicable, based on client criteria and training requirements.

  • Complete regular client visits to evaluate progress, monitor service delivery, and ensure services are implemented in alignment with the Person-Centered or Individual Support Plan and service definitions.

  • Maintain ongoing communication with case managers and care coordinators and ensure complete, accurate, and timely documentation in the client chart, home chart, and computer system.

Field Staff Leadership and Supervision
  • Provide oversight and supervision to Habilitation Technicians in accordance with BAYADA policy, procedure, and state regulation.

  • Participate in hiring, interviewing, orientation, training, coaching, recognition, counseling, and performance feedback for field staff.

  • Serve as a liaison between field and office staff to support coordinated, high-quality care.

Office and Program Support
  • Support office teammates, participate in performance improvement activities, and represent BAYADA in community and marketing efforts.

  • Assist in the development, management, and implementation of Habilitation services and contracts while maintaining positive relationships with referral and funding sources.

  • Provide guidance to office colleagues to promote understanding of and responsiveness to the needs of clients with intellectual and developmental disabilities and related conditions.

Qualifications
  • Demonstrates the characteristics of The BAYADA Way: compassion, excellence, and reliability.

  • Prior supervisory experience required.

  • Demonstrated record of goal achievement, strong interpersonal skills, and success in taking on increasing responsibility.

  • Competence in basic PC skills and the ability to read, write, and communicate effectively in English.

  • Must successfully complete all required pre-employment assessments and background checks in accordance with BAYADA and state requirements.

Hawaii-Specific Requirements
  • Must have a bachelor’s degree from an accredited college/university or Positive Behavior Supports training provided or certified by the Hawaii Department of Developmental Disabilities.

  • Must complete continuing education and/or training in Positive Behavior Supports at least once every two years.

  • In Hawaii, a Habilitation Manager may serve as a qualified supervisor.

  • Must perform face-to-face observation and supervision at least monthly, or more frequently as indicated in the client’s ISP. Reviews may be conducted in person or via Clinical Zoom when agreed upon and documented in the ISP.
