Job Overview:

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our home health care team as a Case Manager. The Home Health RN Case Manager will provide high-quality nursing care and case management services to patients in their homes, coordinating their treatment plans and collaborating with physicians, caregivers, and other healthcare professionals. This field-based role requires excellent clinical skills, strong communication abilities, and a commitment to providing exceptional care to patients in a home setting.

Key Responsibilities:

  • Patient Assessment: Conduct comprehensive home health assessments, including the evaluation of the patient’s physical, emotional, and social needs. Develop individualized care plans based on assessment findings and in collaboration with the physician and interdisciplinary team.
  • Care Coordination: Coordinate all aspects of patient care, including scheduling visits, obtaining medical orders, and collaborating with the patient’s care team to ensure effective communication and continuity of care.
  • Patient Education: Educate patients and their families about their conditions, treatment plans, medications, and self-care techniques. Empower patients to actively participate in their care.
  • Ongoing Monitoring: Monitor patient progress, adjusting care plans as needed. Document changes in the patient’s condition and notify physicians or other team members of significant developments.
  • Medication Management: Ensure proper medication management and compliance, including administering medications, monitoring for side effects, and providing education on proper use.
  • Clinical Documentation: Maintain accurate and timely documentation of patient care, assessments, treatments, and outcomes in accordance with agency policies and regulatory requirements.
  • Compliance & Safety: Adhere to all regulatory standards, policies, and procedures related to home health care. Ensure patient safety at all times by identifying risks and implementing preventive measures.
  • Family Support: Provide emotional support and guidance to families of patients, addressing any concerns or challenges they may face in the care process.
  • Collaborative Approach: Work closely with physicians, physical therapists, social workers, and other healthcare professionals to create and execute comprehensive, patient-centered care plans.

Qualifications:

  • Education: Associate’s degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program.
  • Licensure: Current and valid RN license in the state of Indiana.
  • Experience: Minimum of 2 years of nursing experience, with at least 1 year of experience in home health care or case management preferred.
  • Skills:
  • Strong clinical assessment and critical thinking skills.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work independently and manage multiple tasks.
  • Proficiency in electronic health records (EHR) and Microsoft Office Suite.
  • Certifications: CPR certification required. Additional certifications such as Home Health Nursing or Case Management are a plus.

Working Conditions:

  • Field-based Role: The RN Case Manager will travel to patient homes within the designated service area. Will work from the office.
  • Physical Requirements: Must be able to lift and assist patients as necessary. Must also have the ability to stand, bend, and walk for extended periods.
  • Schedule: Full-time position with flexible hours, including potential on-call or weekend work, depending on patient needs.
