About the Opportunity
NPORT is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our Rehabilitation and Nursing team located in the heart of Orange County, NY. This position provides the opportunity to work in both Home Care (CHHA) and Skilled Nursing settings, offering a dynamic and rewarding experience caring for patients across different levels of rehabilitation needs.
The PTA will primarily provide therapy services to home care patients within the community while also assisting with weekend coverage and additional support in the nursing facility as needed. This role is ideal for a flexible, dependable, and patient-focused clinician who enjoys working independently in home care while remaining part of a collaborative interdisciplinary rehabilitation team.
Responsibilities

  • Provide Physical therapy treatments under the direction and supervision of a licensed Physical Therapist
  • Deliver quality therapy services to patients in home care (CHHA) and skilled nursing environments
  • Assist patients in improving mobility, strength, balance, endurance, and functional independence
  • Accurately document patient treatments, progress notes, and outcomes in a timely manner
  • Communicate effectively with therapists, nursing staff, physicians, patients, and families
  • Monitor patient response to treatment and report changes to the supervising Physical Therapist.
  • Maintain compliance with all state, federal, facility, and regulatory guidelines
  • Provide weekend coverage and additional support within the nursing facility as needed

Qualifications:

  • Current New York State PTA license required
  • Graduate of an accredited Physical Therapist Assistant program
  • Previous experience in skilled nursing, rehabilitation, or home care preferred
  • Strong communication and organizational skills
  • Ability to work independently and manage time efficiently
  • Reliable transportation for home care visits
  • Compassionate, professional and team-oriented attitude
