Responsible for assisting the Residential Coordinator in the care, safety and well-being of persons receiving services living in assigned residences.

POSITION SUMMARY:
Responsible for assisting the Residential Coordinator in the care, safety and well-being of persons receiving services living in assigned residences. Assists in setting performance standards and providing the support and training necessary for direct care staff to achieve standards. Assists in the evaluation of direct care staff's performance and provides feedback, guidance and/or discipline, as appropriate. Assists the Program Coordinator in the development, implementation and monitoring of each program plan and for ensuring that all persons receiving services are afforded opportunities to participate in activities that promote individualization, independence, inclusion and, productivity.
POSITION DUTIES:
  • 1. Responsible for direct supervision of all day Direct Care staff and all daily routines of the day shift. Schedules routine Direct Care staff meetings for the unit on the day shift.
  • 2. Monitors all time and attendance records for all Direct Care staff on the day shift in assigned residences.
  • 3. Interviews, hires, evaluates, supervises, and disciplines day shift Direct Care Counselor staff in assigned locations. Conducts face-to-face meetings with individual day shift staff and documents such.
  • 4. Assists Residential Coordinator and Program Coordinators in initial orientation of new day shift personnel as well as with routine on-going training. Attends all scheduled program unit meetings.
  • 5. Ensures adequate staffing for evening shift when necessary in conjunction with Deployment Supervisor(s).
  • 6. Ensures that all day shift fire drills are completed and documented as required. Identifies any safety concerns to Residential Coordinator.
  • 7. Completes and documents weekly physical site inspections and generates purchase orders and work orders as needed. Communicates all necessary information to the Residential Coordinators and Program Coordinators.
  • 8. Ensures Programming Data Collection is completed prior to the end of shift.
  • 9. Ensures that all food and household supplies are adequate and maintained weekly.
  • 10. Completes a review of all financial ledgers at least weekly to ensure that expenditures are appropriate and that receipts are maintained.
  • 11. Ensures that all needed paperwork, i.e. CVS, and adaptive equipment are sent with the service recipient for all medical appointments.
  • 12. Observes breakfast meals weekly (per site) and provides training as needed to all staff.
  • 13. Responsible for ensuring that all preparation is done for both cleaning and extermination of sites. Ensures housekeeping team completes all necessary cleaning and documents same on housekeeping cleaning form. Conveys any problems noted.
  • 14. Works closely with the Program Coordinator to ensure services are provided and ensures appropriate activities are planned and implemented.
  • 15. Performs all other duties as assigned.

REQUIREMENTS:
  • 1. Associate's degree preferred in Human Services/or related field or, three to five years experience working in the ID/DD field in lieu of formal educational qualifications.
  • 2. Knowledge of HCBS Waiver IRA and ICF regulations preferred.
  • 3. Must be able to work a 6:00am-2:00pm shift daily and work flexible hours as needed, be on-call, and be able to provide weekend coverage as scheduled.
  • 4. Working knowledge of Microsoft Applications.
  • 5. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency's corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
