As a Social Services Agent, you will play an important role in helping individuals and families access essential government assistance programs and community resources. You will assist clients with applications for benefits, provide guidance throughout the eligibility process, and help connect patients with the services they need to improve their health and well-being. Key Responsibilities

• Educate clients, patients, and visitors on the ACCESS application process and available assistance programs
• Explain the role of the Social Services Agent and available support services
• Complete required Department of Children and Families (DCF) and Community Medical Group consent forms
• Assist clients with applications for Food Stamps, Medical Assistance, Temporary Cash Assistance, and Medicare Savings Programs
• Help clients create and manage their My ACCESS Account
• Monitor and follow up on ACCESS applications until eligibility determinations are made
• Notify clients regarding application decisions and eligibility outcomes
• Educate eligible Medicaid beneficiaries on their health plan options and primary care provider selection process
• Assist patients with health plan enrollment and selection activities
• Document client demographics, enrollment activities, and application progress accurately within company systems
• Provide referrals to community agencies and support services when appropriate
• Assist clients with access to courtesy phone services as needed
• Maintain accurate and confidential client records and documentation
• Ensure compliance with HIPAA regulations and company privacy standards
• Handle all applications and client information in an ethical, professional, and respectful manner
• Perform additional duties as assigned

Qualifications

High school diploma or equivalent required
• Previous customer service experience required
• Experience working with DCF programs, ACCESS applications, or other government assistance programs preferred
• Experience with data entry and client management systems preferred
• Proficiency with Microsoft Office applications required
• Strong communication and interpersonal skills
• Excellent organizational and time-management abilities
• Strong attention to detail and accuracy
• Ability to exercise sound judgment and maintain confidentiality
• Strong problem-solving and decision-making skills
• Ability to work independently and collaboratively within a team environment
• Commitment to providing exceptional customer service and community support
Bilingual in English and Spanish or English and Creole preferred
