As part of the person-centered care model, this role will participate in the daily management and delivery of wound care for the Wound program. This role will aid in the delivery of outcome based wound care, cost effective care by utilizing metric-driven decisions. This role will collaborate with Wound team RN and CWOCN to achieve exceptional outcomes. The Wound team LPN performs wound care in the daily delivery of services and maintenance of clinical care standards for clients receiving specialized wound care treatments. Works with the Wound Team RN, CWOCN and Manager of Specialty services to promote the daily operations and delivery of wound care to patients, and participates in the development, maintenance and implementation of program goals and objectives.
Participates in ensuring consistent delivery of quality care and communications; Performs wound care to promote cost effectiveness for wound treatments in relationship to patient needs
Seeks consultation with the Wound Team RN and WOCN staff for patient problems/issues that require a change in the plan/interventions designed to solve complex problems involving client care.
Participates in the interdisciplinary approach to client care with Wound program/Agency staff. The Wound Program LPN will perform both Wound care and medical/surgical services Care required by the patient to achieve quality outcomes. Informs the Wound Team RN, CWOCN and the physician/wound clinics of changes in client needs, wounds that require a change to the plan of care for wound care treatments and disease management
Assists with staffing needs to meet service demands.
Accountable for team performance in achieving desired clinical and operational performance measures.
Follows trends in specialty area of wounds for home care industry; makes recommendations of treatment options for incorporation into the program.
Participates in training, coaching of staff or onboarding to the Wound program
Participates and facilitates professional development needs and competency for wound program.
In addition, the position involves other duties such as:
1. Will actively participate in the performance improvement process known as H3W.
2. Participates in clinical education area to provide programs aimed to improve clinical competencies of direct service staff.
3. Research new information related to wound specialty and communicates to team.
4. Performs other duties as assigned.
Qualifications:
Education
CT approved Licensed Practical Nursing program required. Wound/Ostomy Assistant certification preferred.
Experience
Minimum of two years nursing experience in Clinical specialty area required. Two years in homecare preferred.
