Position Overview

The Practice Success Partner is the single accountable owner of the relationship between Grace At Home and an assigned panel of practices participating in value-based care contracts. This person is a data-driven performance consultant and relationship owner. The Practice Success Partner uses contract performance data to set the practice's overall action agenda, owns the practice's relationship end to end, and is the practice's primary point of contact.

Key Responsibilities Performance Targeting
  • Translate contract performance data (from Healthcare Economics and Enterprise Analytics) into a specific, practice-level action agenda — what to target, in what order, and why.
  • Maintain a focused set of priority actions per practice, reviewed with practice leadership on a fixed cadence, rather than spreading attention evenly across every possible workstream.
  • Prioritize time and attention toward the highest-concentration practices driving the largest share of the panel's performance gap.
Leadership Relationship Ownership
  • Serve as the single accountable point of contact for each assigned practice — the owner of the relationship, not one of several contacts.
  • Own practice satisfaction and the escalation path; resolve issues directly where possible and escalates to the Regional Operations Leader where it requires leadership intervention.
Workflow Implementation
  • Drive practice adoption of the workflows that move contract performance: transitions-of-care follow-up, Core Four quality measure closure, and Care Pathway engagement.
  • Coach the practice on how to embed these workflows into its own operations rather than executing them indefinitely on the practice's behalf.
Cross-Functional Coordination
  • Take direction from the enterprise Quality and Risk Adjustment teams, who set strategy and targets; flag practice-level gaps to them and drive practice adoption of the resulting action items. Does not perform chart review, coding audits, or other chart-level clinical work.
  • Partner with the Regional CMO on clinical standards for practice-facing work; escalates clinical-judgment matters to the Regional CMO rather than making those calls independently.
  • Coordinate with the centralized Network team on administrative items — contracting, NPI/TIN association, credentialing — without owning them.
Required Qualifications Qualifications Required
  • Five or more years of experience in a practice-facing, account management, or provider relationship role within a value-based care, managed care, or provider organization.
  • Demonstrated ability to read a contract performance report and convert it into a specific, prioritized action plan.
  • Demonstrated ability to own a leadership-level relationship, including direct conversations with practice leadership and physicians.
  • Working knowledge of value-based care contract mechanics: quality measures, risk adjustment, transitions of care, and medical expense drivers.
  • Proficiency with Excel and standard office software; ability to learn payer portals, analytics platforms, and internal tracking tools.
Preferred
  • Prior experience in an account management, network management, or practice transformation role that combined relationship ownership with performance accountability.
  • Experience with HEDIS/Stars quality programs or risk adjustment coding processes (from a coordination standpoint, not chart-level execution).
  • Bachelor's degree or equivalent experience.
Competencies
  • Prioritization. Manages to a small number of priority actions and dates per practice rather than diluting attention across every initiative at once.
  • Relationship gravity. Carries credibility with practice executives and physicians; owns hard conversations without escalating every issue.
  • Boundary discipline. Understands and respects the boundary between this role's scope and Quality/Risk Adjustment, Network, and the Regional CMO — coordinates rather than duplicates.
  • Judgment on escalation. Knows when a practice issue is coaching, when it is capacity, and when it requires escalation to the Regional Operations Leader.
  • Independent judgment. Works independently across a multi-practice panel without daily direction.
The working environment and physical requirements of the job include:

In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.

In this position you will need an to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
