Job description

CSI may have one or more Healthcare Coordinators who shall function under the supervision of the Director of Nursing Services (RN). The role of the Healthcare Coordinator is to assist in achieving CSI’s mission by ensuring that the individuals supported by CSI receive high quality health services. This includes ensuring early evaluation of health/dental needs, timely implementation of required treatment and the review of CSI policies and/or procedures to assure that the services rendered meet or exceed all applicable standards of care. This position is responsible for assisting the Director of Nursing Services in implementation of CSI policies and procedures, quality assurance review, as well as supervision/training of licensed and non-licensed staff.
JOB SPECIFICATIONS:
The position of Healthcare Coordinator - RN requires that the employee be licensed in the state of practice as a Registered Nurse with at least two years experience working with people who have intellectual disabilities preferred. Other required qualifications include: excellent written and oral communication skills, computer/word processing skills, CPR certification and physical management techniques certification. This position requires a valid driver’s license in the state of practice with a driving history free of significant infractions within the past 7 years.
ESSENTIAL RESPONSIBILITIES, DUTIES AND TASKS:
Duties of the Healthcare Coordinator – RN shall include, but are not limited to:

  • Provide overall healthcare coordination to persons supported, to include establishing their health profiles, identifying their health risks, identifying and establishing relationships with appropriate healthcare providers, providing liaison to those providers, and other tasks as described further below.
  • Ensure that persons supported have the opportunity to choose healthy lifestyles, to include proper diet and nutrition, exercise, and other preventive health measures as appropriate to each individual. From a health perspective, ensure that the CSI mission is fulfilled…..helping persons with disabilities find happiness in their own homes, in their personal relationships, and as a contributing member of their community.
  • Respond to requests from staff to assess a person’s health status within a timeframe appropriate to the nature of the request. The appropriate response may involve telephone communication with the staff, an on-site visit to the person supported, or meeting CSI staff upon the person’s arrival at the hospital to seek evaluation an/or emergency treatment by hospital physicians.
  • Transcribe medical orders received from physicians, assuming necessary liaison with the pharmacy and, subsequently, with CSI staff to ensure accuracy and timely implementation of the orders.
  • Participate as a member of IDTs, when appropriate, as one of CSI’s representatives on behalf of a person supported.
  • Identify the health related training needs of non-licensed staff. Train, coach, and certify that staff has been trained on nursing delegation tasks, as appropriate. These tasks may include assisting with medication (passing and pouring medication), the taking of vital signs, feeding tube utilization, and other tasks which can be delegated. Train and coach staff in other health-related topics to include nutrition and diet, among others. (Note: non-licensed staff in MA, once duly trained, act independently of a nurse’s license.)
  • Oversee the creation and maintenance of persons’ medical records to ensure compliance with state and internal regulations and policy. Monitor the records to ascertain that persons are receiving appropriate and timely healthcare consults, evaluations, and follow-up treatment. Monitor assistance with medication (passing and pouring medication) for persons supported in a manner which promotes accuracy in dosage, method of delivery, timeliness, and record keeping by staff.
  • Monitor CSI-supported homes and apartments for safe and sanitary conditions and infection control, with special attention to medication storage, kitchen practices, food storage, storage of cleaning supplies, handling of soiled linens, etc.
  • Participate in health-related problem-solving task forces and committees at the direction of the Executive Director. Provide recommendations and follow-up staff training as appropriate.
  • Maintain proficiency and currency in the latest best practices in the nursing profession, including the relevant scope of practice and standards of care. Have knowledge of and compliance with the principles of delegation, and participate in the delegation process as directed by the Director of Nursing Services.
  • Share in the preparation of homes and apartments for licensure and other survey visits. Assist in the preparation of CSI responses to citations, if any.
  • Provide direct care, if indicated, such as to accompany a person supported with staff to a complex medical appointment.
  • Perform other related duties as may from time to time be assigned by the Director of Nursing Services.
