At Crest Healthcare Group, we are seeking an experienced Hospital Liaison to join our dynamic team. As a Hospital Liaison, you will be responsible for developing and maintaining strong relationships with referral sources and community partners, while driving sales strategies to increase guest/resident admissions. Your role will focus on the rehabilitation of medically complex patients who require skilled nursing services, and you will have the opportunity to make a significant impact on the lives of seniors.

Your Role:

  • Build and nurture long-term relationships with referral sources, including physicians, social workers, case managers, and insurance coordinators.

  • Spend time in the field, assessing patients and enhancing referral relationships through hospital visits and community events.

  • Educate guests and families on the benefits of Crest Healthcare's services, acting as a liaison during the transfer process.

  • Collaborate with the DON and ADON to develop initial care plans and ensure smooth transitions for patients.

  • Work with the Admissions Coordinator to provide daily updates on patient admissions and collaborate on reporting using internal and external data.

  • Represent Crest Healthcare Group at community events and promote the mission of the organization.

Who We’re Looking For:

  • Previous experience as a Hospital Liaison with a strong network in Long-Term Care is preferred.

  • Multi-facility experience is a plus!

  • Strong communication skills, with the ability to engage diverse nursing and community care staff.

  • Ability to achieve high levels of customer satisfaction and meet referral targets.

  • Professional, dedicated, and knowledgeable about skilled nursing services and healthcare.
