Through education, care navigation, and joyful community engagement, OCC helps families navigate aging with dignity and connection. OCC specializes in dementia care while serving a broader aging population.


Position Summary

The Hub Coordinator serves as a trusted partner for older adults, caregivers, and families while ensuring Oakwood Creative Care's Family Support Hub remains a welcoming, organized, and vibrant center for connection, education, and support. This role combines compassionate care navigation with community engagement and operational coordination, providing dementia-informed education, caregiver support, resource navigation, scheduling, and program coordination. Working collaboratively with Oakwood's interdisciplinary team, the Coordinator helps families navigate aging with confidence while creating meaningful experiences that promote resilience, connection, and well-being.


Care Navigation, Family Support & Hub Coordination

  • Serve as a compassionate first point of contact for older adults, caregivers, and families seeking support, providing guidance, education, and connection throughout their aging journey.
  • Conduct comprehensive intake and psychosocial assessments to identify family strengths, needs, risks, and goals.
  • Support families in understanding dementia, cognitive changes, disease progression, behavioral symptoms, and available care options.
  • Provide individualized coaching on communication strategies, behavioral approaches, safety planning, caregiver resilience, and long-term care planning.
  • Assist families through significant care transitions, including hospital discharge, adult day enrollment, memory care placement, hospice, and end-of-life planning.
  • Connect families with appropriate medical, legal, financial, and community resources while helping them navigate complex healthcare, long-term care, and community-based service systems.
  • Provide crisis support, problem-solving, and emotional guidance during periods of caregiver stress or behavioral escalation.
  • Coordinate care navigation services, including scheduling appointments, preparing educational materials, coordinating referrals, tracking provider orders, and facilitating communication between families, healthcare providers, and Oakwood's interdisciplinary team to ensure timely follow-up and continuity of care.
  • Support participants across multiple care coordination, clinical, and community-based programs while maintaining seamless communication and continuity throughout their experience with Oakwood Creative Care.
  • Maintain accurate documentation, care plans, service records, and outcome data while supporting quality improvement, compliance, and reporting requirements.
  • Participate in a rotating 24/7 caregiver support hotline to provide timely guidance and support to families.

Guest Experience & Hub Operations

  • Create a warm, welcoming, and dementia-inclusive environment that reflects Oakwood Creative Care's mission and values.
  • Serve as the primary point of contact for visitors, families, partners, and community members, providing concierge-level customer service.
  • Conduct tours and educate prospective participants and families about Oakwood Creative Care's programs and services.
  • Answer phones, respond to inquiries, and connect individuals with appropriate resources and team members.
  • Coordinate daily Hub operations, including calendars, appointments, registration, check-in, communication, and administrative workflows to ensure a seamless and supportive experience.
  • Foster an environment where older adults, caregivers, and community members feel connected, valued, and supported.

Caregiver Education & Community Programming

  • Facilitate caregiver support groups and educational workshops
  • Coordinate and support community programming, including Memory Cafes, Mix & Mingle gatherings, healthy aging workshops, family events, memory screenings, and other educational and social engagement opportunities.
  • Assist with event planning, room setup, registration, guest experience, and logistics to ensure high-quality, welcoming programs.
  • Promote caregiver resilience, education, social connection, and access to resources through innovative, person-centered programming.

Collaboration & Professional Excellence

  • Work collaboratively with Oakwood's interdisciplinary team, healthcare providers, referral partners, and community organizations to ensure coordinated, person-centered care.
  • Participate in interdisciplinary meetings, case consultations, and ongoing professional development to remain current on best practices in dementia care, aging services, caregiver support, and care coordination.
  • Maintain confidentiality and uphold professional, ethical, and regulatory standards.
