You will mentor and assist people at home and in the community incorporating a variety of recreational activities in support of personal goals, daily living skills, and increased independence. Salary Range:ย 

Community Habilitation Specialist I $17.90 - $18.90 per hour

Community Habilitation Specialist II $18.16-19.16 per hour

Community Habilitation Specialist III $18.41-19.41 per hour

ย **Weekend Shift Differential of $1.00 per hour for hours ย worked Friday 11pm- Monday 7am **ย 

Position Standards

  • Training in independent performance of personal hygiene tasks such as appropriate toileting, bathing, tooth brushing, hand and face washing, shaving, and use of deodorant and mouthwash.
  • Training in independent performance of self-care tasks such as eating and use of appropriate dining utensils, dressing and use of grooming aids such as hairbrushes, combs, nail clippers, razors, etc.
  • Training in independent performance of general household chores such as meal preparation, routine shopping, laundry, cleaning, and maintenance of an orderly environment.ย 
  • Mobility training for maximum independence in travel to and from work or activity center, community stores, recreational sites, etc. It included training in the use of public transportation.
  • Training to maximize independence in personal health care such as regular medical and dental appointments, basic first aid skills, self-medication, where appropriate, and basic knowledge of proper nutritional habits.
  • Training to maximize personal financial management such as recognition and counting skills, use of money for over the counter purchases, and change making.
  • Training in appropriate social behaviors which are normative in both the community and household settings such as conducting oneself appropriately in restaurants, on public transportation vehicles, in recreational facilities, and in stores and other public places.
  • Follow community habilitation staff action plan and record and monitor progress.
  • Meet regularly as per oversight agencies and service program requires.
  • Submit billing documentation to supervisor in a timely manner.ย 
  • Assume driving responsibilities when requested and transport participants as necessary.
  • Fulfills mandated reporter role by initialing protective measures, as needed and reporting suspected abuse to administration immediately.ย 
  • Ensures all required trainings and certifications required are current, and seeks further support and training where needed.
  • Adhere to agency Code of Legal and Ethical Behavior, which require all employees, consultants, board members, volunteers and affiliates to perform their responsibilities according to ethical and legal standards with honesty, integrity, fairness, good faith and respect for others and the law.
  • Follows all agency policy and procedures and demonstrates continuous regard to personal safety and the safety of others.
  • Performs other appropriate job-related activities as requested by your supervisor or as circumstances warrant.
  • Will incorporate CORE concepts of person-centered excellence by respecting people's concerns and responding accordingly and will use "teachable moments" to assist people in understanding and exercising their rights.

ย Job Requirements:ย ย 

  • High School Diploma or equivalent is required, experience preferred. A combination of education and experience which would be considered by the employer as appropriate to qualify for the position may be acceptable.
  • Must have a current NYS driver's license deemed "acceptable" by our insurance carrier.

  1. Have the ability to lift up to 50 lbs., bend, squat, bend the knees, pivot and grasp with both hands (in order to properly and safely perform the techniques taught in CPR/First Aid, behavior intervention techniques, and lifting and carrying techniques).
  2. Lift individuals by using appropriate techniques, including but not limited to usage of tools to assist, such as the Hoyer Lift.
  3. Assist individuals in ambulation by providing physical assistance, including use of a gait belt or other equipment where necessaryย 
  4. Have the ability to stand and walk for extended periods of timeย 
  5. Have the ability to push and pull individuals in wheelchairs.
  6. Possess verbal and written communication skills, in English, to ensure adequate regulatory documentation.
