Home Health & Hospice Aide (HHA/CNA) – PRN | Bend, OR

💰 $24.00 – $28.00/hour (DOE) + Mileage Reimbursement

Signature Bend Home Health & Hospice is seeking a dedicated Home Health & Hospice Aide (HHA/CNA) to provide essential personal care and support services to patients in their homes.

This PRN (as-needed) role is ideal for caregivers who are passionate about patient-centered care, hospice support, and improving quality of life for individuals facing illness, recovery, or end-of-life needs.

🔎 What You’ll Do

Patient Care & Daily Living Support

  • Assist patients with personal care, including bathing, grooming, hygiene, and dressing
  • Support toileting, mobility, and transfers
  • Provide oral care, skin care, and repositioning to prevent breakdown
  • Assist with meal preparation, feeding, and light housekeeping
  • Perform range of motion exercises and activity support per plan of care

Health Monitoring & Reporting

  • Observe and report changes in patient condition to the RN or therapist
  • Take and document vital signs (as trained and permitted)
  • Maintain accurate, timely documentation of care provided

Team Collaboration

  • Work under supervision of a Registered Nurse (RN) or therapist
  • Participate in the interdisciplinary care team (IDG)
  • Attend case conferences and in-service training

What You Bring

  • Current Home Health Aide (HHA) certification or Certified Nursing Assistant (CNA) in Oregon
  • Completion of a state-approved training and competency program
  • Minimum age of 18 years
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and auto insurance
