PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Lagrange, Kentucky.

Position Overview

We are seeking a dedicated Registered Nurse to provide high-quality home health care to patients across multiple counties. This role focuses on delivering skilled nursing services, completing OASIS assessments, and ensuring comprehensive patient care in a home setting. The ideal candidate brings strong clinical skills, attention to detail, and an ability to work independently in a field-based environment.

Schedule & Work Details

  • Location: Coverage across Jefferson, Oldham, Henry, Trimble, and Carroll counties
  • Shift: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hours: 32-hour guarantee
  • On-Call/Weekend Requirements: One weekend per month and one weekday per week
  • Mileage Reimbursement: Yes, reimbursed between patient visits from first to last patient
  • Pay Rate: $51 to $53 per hour

Key Responsibilities

  • Provide skilled nursing care to patients in a home health setting
  • Complete accurate and timely OASIS assessments and documentation
  • Monitor patient conditions and update care plans as needed
  • Educate patients and families on disease management, medications, and care plans
  • Collaborate with interdisciplinary care team members to ensure continuity of care
  • Maintain compliance with agency policies, regulatory requirements, and best practices
  • Manage a caseload efficiently while maintaining quality and patient satisfaction

Required Qualifications

  • Active Registered Nurse (RN) license
  • Minimum of 1 year of nursing experience
  • Ability to work independently in a field-based home health setting
  • Strong clinical assessment and documentation skills

Preferred Qualifications

  • Homecare Homebase (HCHB) experience
  • Prior home health nursing experience
  • OASIS documentation experience
  • Strong hands-on patient care background and time management skills
