Job Summary:

Seeking an experienced Home Health RN to provide skilled nursing visits for adult patients in home care settings. The RN will perform assessments, wound care, IV therapy, patient education, and post-operative care while managing independent home visits.

Key Responsibilities:
  • Perform skilled home health visits (6–8 visits per day)
  • Conduct cardiopulmonary assessments and patient evaluations
  • Provide wound care, IV therapy, PICC/Midline care, injections, and catheter care
  • Educate patients and families regarding medications, disease management, and safety
  • Complete documentation using BayCare Forms and Ace Timesheet system
Required Skills:
  • Skilled Home Health experience required
  • OASIS training and documentation experience required
  • IV skills and wound care experience
  • Experience with CHF, COPD, MI, Pneumonia, Stroke, and post-op patients
  • Venipuncture, wound vac, glucometer, SQ/IM injections, and line care experience
Requirements:
  • Active RN License
  • Active BCLS Certification
  • Minimum 1 year Home Health experience required
  • Valid driver’s license and reliable vehicle required
  • COVID vaccination required (or approved exemption)
  • Flu shot required or signed declination with mandatory mask use
