Job Description
Skilled Home Health RN
Schedule
Mon-Fri, with typical shift windows of 8:00 AM to 5:00 PM or 9:00 AM to 6:00 PM. Occasional Thursday-Monday or Friday-Tuesday shift blocks. Weekend coverage required on alternating weekends.
Assignment Details
6-8 skilled visits per day.
Requirements
  • Minimum of 1 year recent clinical experience.
  • Home health experience with OASIS training and documented OASIS use is required. Private-duty-only experience is not accepted.
  • FL license or compact license required.
  • Current BLS/BCLS certification required.
  • Valid driver's license and personal vehicle required for home visits.
  • Experience with IV start/maintenance, wound care, and cardiopulmonary assessments.
Responsibilities
  • Conduct patient assessments and develop care plans.
  • Administer medications and treatments as prescribed.
  • Perform wound care including wound VAC and dressing changes.
  • Provide education and support to patients and families.
  • Coordinate with interdisciplinary teams to ensure comprehensive patient care.
  • Document patient progress and outcomes in charting systems.

Compliance / Notes
COVID-19 vaccine required; medical/religious exemptions allowed. Flu shot or declination required, with mandatory masking for declination. Compliance documents due 10 days prior to start.
