Enhance your Family Care career in this Manager, Care Coaching role. Reporting to the Associate Director, Care Coaching, you will support the work of Care Management staff. You will provide daily leadership and mentoring to staff. You will deliver services in a manner that expresses Humana values and the Family Care program values of access, choice, quality, and cost effectiveness. This position supports IDT staff that cover Jackson and Trempealeau Counties.

Main Responsibilities:

  • Mentor and support the work of Field Nurses and Care Coaches (or IDT) to ensure the Family Care benefit is delivered in compliance with the DHS contract.
  • Identify learning needs; arrange for mentorship, provide clear expectations, facilitate orientation and ongoing learning to build capacity of IDT staff.
  • Assess and coach staff on the IDT primary competencies and performance standards.
  • Review reports, assess process and review documentation.
  • Assess utilization and the quality of care management for each member served.
  • Participate in cross departmental collaboration to ensure the quality and effectiveness of provider services.
  • Educate community partners.
  • Participate in the hiring process by selecting, orienting, and training associates.
  • Facilitate in person unit meetings and attend quality visits as needed.

Required Qualifications

  • Bachelor's degree in human services or a related field or associate degree in nursing.
  • 1 year of leadership experience in the human services field.
  • 3 years of experience working in the Family Care program.
  • Reside within 1 hour of either Black River Falls or Blair, WI

Preferred Qualifications

  • 5 years of experience working with individuals with intellectual/developmental disabilities, physical disabilities or frail elders.

Additional Information

  • Workstyle: Work remote from home
  • Travel: at least 1 time per month with potential for more
  • Typical Work Days/Hours: Monday - Friday, 8:00 am - 4:30 pm CST
  • Direct Reports: approximately 11-15
Work at Home Requirements: To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


$78,400 - $107,800 per year


This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
