Job description

Case Manager is responsible for coordinating all case management services and activities at St. Michael's Home for Children. Candidate will ensure that weekly case management is provided to the minors in care and efforts are concentrated on family reunification services. Responsible for ensuring that assessment of placement, communication with family members and identified sponsors for release, and after care services are coordinated and implemented. Case Managers primarily lead the treatment team as a critical part of the service delivery team to assure the client's needs are being met through an individualized service plan.
PRINCIPAL DUTIES
  • Maintain a case load in accordance with agency policies and procedures and licensing and contract standards. Maintain a minimum number of weekly contacts with children and families. Conduct assessment and gathering of information in relation to placement appropriateness and family reunification efforts. Risk and Safety factors are taken into consideration to move forward with the discharge plan focusing on family reunification services. Ensures all case management services provided to minors and families in compliance with professional licensing standards, agency standards, Accreditation standards, ORR requirements, and HIPAA. Direct client service delivery includes but is not limited to the following case management duties: conducting assessments in person or on the phone, developing and implementing treatment plans, assessing risk and safety needs for possible sponsors regarding home studies or follow-up services, incident reporting and providing appropriate referrals as needed.
  • Complete documentation of client progress within required timeframes and maintain client records in accordance with regulatory/agency standards.
  • Participate in Case Management activities including but not limited to documentation review, data entry and audits of records. Provide leadership in the weekly treatment team meeting and give recommendations on cases in their caseload. Participate in all supervisions and communicate essential information to supervisors.
  • Maintain effective and professional communication with the treatment team and any outside entity including but not limited to ORR, third party reviewers (GDIT), TDFPS Licensing, consulate officials, OHS/ICE officials, attorneys, and other providers as needed.
  • Ensure that collaborating entities have essential information for processing cases. Case Management activities that require external communication include but are not limited to birth certificate verification, age determination, returnto-country processing, home-study or follow-up providers, independent providers, school districts, or medical providers.
  • Establish processes for maintenance of reliable databases, which include Efforts to Outcome (ETO) and UAC Portal. Ensure that all documentation needed on databases is entered in a timely manner. Adhere to all policies, procedures, and guidelines required by databases.
  • Meet all documentation and case management deadlines as required by program supervisors and federal partners.
  • Engage in regular weekly supervision sessions and communicate all essential case updates to supervisors.
  • Adhere to all safety protocols and participate in regular drills, support evacuation and accountability of minors in emergencies. Identify and escalate safety risks, contribute to behavior support plans and deescalation using trauma informed approaches. Comply with incident reporting procedures and timelines.
  • Respond to and participate in emergency situations, including executing evacuation procedures and other safety protocols.
  • Work a rotating schedule that may include evenings, weekends, holidays, and on call coverage, provide shift support to maintain required staffing ratios. Flex between units/teams to meet program needs, cover peer caseload tasks when other case managers are off duty.
  • Engage in ongoing training and professional development related to child welfare, immigration, and case management best practices. Complete and maintain required certifications (e.g., CPR/First Aid), program orientations, and annual refreshers (e.g., trauma informed care, emergency preparedness, confidentiality, child welfare best practices).
  • Performing other duties as required by supervisor.

MINIMUM QUALIFICATIONS
  • Bachelor's degree in the behavioral sciences, human services or social services fields.
  • One (1) year experience in child welfare/residential and/or adolescents in a social service setting.
  • Bilingual in English and Spanish.
  • Valid Texas Driver's License

SKILLS REQUIREMENTS
  • Strong organizational and planning skills.
  • Ability to work well within a team approach.
  • Ability to develop successful working relationships with a variety of people, i.e., staff, senior management, clients.
  • Strong verbal and written communication skills, including command of English language.
  • Proficiency in MS Office Suite (Word, Excel, Access)
