Job Summary:

The Non-Clinical Nurse Case Manager is responsible for providing case management and care coordination for assigned Service Members within the Army National Guard. This position supports medical readiness and administrative case tracking by maintaining accurate documentation, facilitating care plans, and ensuring timely processing of medical information in accordance with Army regulations and standard operating procedures.


Key Responsibilities:Case Management & Documentation
  • Maintain accurate medical documentation and charting on assigned Service Members in eCase (IAW AR 40-66 and unit SOPs).

  • Track medical status and Service Member contact (email, calls, interviews).

  • Document all case management notes and scanned medical documentation in MEDCHARTS eCase and Health Readiness Reporting (HRR).

  • Add medical records to the Soldier Treatment Record (STR) in paper form following HIPAA and PII laws.

Medical Readiness Support
  • Provide timely reports and documentation on Service Members' medical readiness to DSS and unit command.

  • Coordinate treatment plans with medical facilities, schedule required exams/evaluations, and manage automated referrals per SOP.

  • Attend Periodic Health Assessment (PHA) events as needed.

  • Track compliance and non-compliance of Service Members referred by providers.

Care Coordination
  • Coordinate end-to-end case management from condition identification to medical readiness or administrative separation.

  • Obtain Release of Information (ROI) from Service Members.

  • Monitor and document appointment attendance, follow-up care, and lab result reviews.

  • Educate Service Members on available medical/dental resources, TRICARE benefits, and VA eligibility.

Profile and Documentation Management
  • Build initial temporary profile shells based on civilian provider documentation in E-PROFILE within 5 business days.

  • Compose Narrative Summaries (NARSUMs) within 10 days of profile determination (IAW AR 25-50).

  • Transpose treatment plans into various packet types (ADOP, INCAP, Convalescent leave, Medical Board).

  • Follow up with providers as needed to clarify treatment plans.

Collaboration & Compliance
  • Collaborate with Medical Readiness NCOs and military providers to inform leadership of updates and case progress.

  • Ensure adherence to HIPAA standards and Army policies regarding Individual Medical Readiness (IMR).

  • Review and organize documentation for Disability Evaluation System medical board packets according to regulatory guidance.

Reporting & Metrics
  • Maintain and submit monthly reports to the COR including detailed case tracking by name, condition, and unit (UIC).

  • Document medical status, profile needs, medical board referrals, return-to-duty recommendations, and deployability assessments.

Qualifications:
  • Registered Nurse (RN) License (State-specific or compact license as applicable)

  • Prior case management experience preferred, ideally in a military or government setting

  • Familiarity with AR 40-66, AR 25-50, HIPAA, and military medical documentation systems (e.g., MEDCHART, eCase, E-PROFILE)

  • Strong organizational and communication skills

  • Ability to work under pressure and meet tight deadlines

  • Experience handling sensitive medical information and maintaining confidentiality
