Job description

RN Case Manager

The RN Case Manager coordinates management of care and ensures optimum utilization of resources, service delivery, and compliance with external review agencies. This role provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. Enhancing the quality of patient management and satisfaction is a priority, promoting continuity of care and cost-effectiveness through the integration of functions of case management, utilization review, and discharge planning. The RN Case Manager also serves as a patient advocate, addressing the holistic needs of the patient through resource utilization and discharge planning.

  • Coordinate care management and resource utilization.
  • Develop and implement individualized care plans.
  • Conduct comprehensive assessments and evaluations of patient needs.
  • Support effective discharge planning and continuity of care.
  • Advocate for patients' holistic needs and preferences.
  • Collaborate with interdisciplinary teams to enhance service delivery.

This position is scheduled Monday through Friday, 8:00 AM to 4:30 PM.

  • Must have a current Illinois RN license.
  • Minimum of 2 years of recent RN Case Management experience, preferably in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting.
  • Strong collaboration with interdisciplinary teams (IDT) and experience in discharge planning across acute care settings.
  • MCG knowledge is preferred.
  • Basic Life Support (BLS) certification from the American Heart Association is required.
