What You’ll Do:
  • Assess the psychosocial needs of patients and their environments, sharing findings with the care team.
  • Conduct social evaluations and create intervention plans based on findings.
  • Maintain detailed clinical records for all referred patients.
  • Prepare comprehensive clinical and progress notes.
  • Provide patients and families with resources and referrals to address practical and environmental needs.
  • Deliver care with passion and compassion in every interaction.

Who You Are:
  • Education: Graduate of a master’s program in social work accredited by the Council on Social Work Education.
  • Licensure: Current licensure in the state of Michigan. - Cannot accept Limited License MSW
  • Experience: At least one (1) year of Home Health experience required.
