Job Description:

As a home health nurse at Custom, you'll play a crucial role in empowering our patients to regain independence and improve their quality of life. Your responsibilities will include:

  • Providing skilled care tailored to each patient's needs, facilitating their journey towards independence.
  • Conducting thorough initial assessments to develop personalized care plans in collaboration with physicians and the interdisciplinary team.
  • Educating patients and their families on ongoing care strategies and lifestyle improvements.
  • Coordinating and administering skilled treatments aimed at enhancing patient well-being and overall health.

Qualifications:

  • Valid registered nursing license (required).
  • Home health experience is required.
  • 1+ years of registered nursing experience.
  • Associate degree in nursing (required).
