Description
I. System-Wide Social Services Leadership & Oversight
  • Provide administrative and clinical oversight of all CHCS social services programs.
  • Develop, implement, and update social services policies and procedures in alignment with CMS Conditions of Participation, Kansas regulations, and critical access hospital standards.
  • Ensure compliance with:
    • CMS Long-Term Care regulations
    • F-tags related to behavioral health and psychosocial well-being (including F699, F741)
    • PASARR requirements
    • Kansas Administrative Regulations (K.A.R. 28-39-153)
  • Monitor staffing ratios to ensure compliance with required social services hours per resident day.
  • Conduct internal audits of documentation, care plans, MDS sections, and trauma-informed assessments.
  • Provide monthly documented chart reviews for long-term care residents to demonstrate licensed social worker oversight.
  • Ensure quarterly and annual psychosocial assessments are completed per regulatory standards.
  • Provide written supervisory summaries documenting:
    • Length of supervision sessions (minimum 4 hours/month per supervisee)
    • Client types and case themes discussed
    • Goals and objectives of supervision
  • Maintain required written supervision agreements with non-licensed social service providers.
  • Budgetary preparation annually with Director insight.
  • Growth and development of social services and behavioral health services.

II. Behavioral Health Program (Clinical Services)
  • Provide outpatient mental health counseling services.
  • Conduct comprehensive mental health assessments.
  • Develop and implement individualized treatment plans.
  • Provide individual and/or group therapy.
  • Conduct suicide risk assessments and crisis intervention.
  • Maintain compliant documentation of all behavioral health encounters.
  • Collaborate with primary care providers within rural health clinics.
  • Evaluate effectiveness of counseling programs and outcomes.
  • Assist in developing and expanding integrated behavioral health services.

III. Mental Health First Aid & Community Outreach
  • Serve as CHCS lead for Mental Health First Aid (MHFA) programming.
  • Provide or coordinate MHFA training for:
    • CHCS staff
    • Schools
    • Community partners
  • Develop partnerships with local school districts and community organizations.
  • Establish Memorandums of Understanding (MOUs) and service agreements.
  • Provide educational presentations on:
    • Suicide prevention.
    • Trauma-informed care.
    • Behavioral health awareness.
  • Identify community mental health needs and assist in program development.

IV. Mental Health Task Force Leadership
  • Co-chair the CHCS Mental Health Task Force alongside the Director of Ancillary Services.
  • Facilitate meetings and strategic planning initiatives.
  • Develop system-wide behavioral health improvement goals.
  • Track progress and quality metrics.
  • Coordinate cross-departmental collaboration to address behavioral health needs.

V. Quality Improvement & Program Development
  • Participate in Quality Improvement initiatives.
  • Analyze psychosocial and behavioral health trends.
  • Develop new service lines or program enhancements.
  • Ensure services align with CHCS mission and rural health needs.
  • Provide staff education on:
    • Trauma-informed care
    • Dementia care best practices
    • Non-pharmacological behavioral interventions

I. Long-Term Care Social Services Oversight (CMS Compliance)
Serve as Licensed Social Worker providing required supervision of Social Services Designee(s) to ensure the following criteria are being met:
  • Review Social History Assessments upon admission.
  • Ensure quarterly psychosocial assessments are completed during MDS review.
  • Oversee trauma-informed care assessments at admission and as indicated.
  • Audit:
    • Care plans for behavioral and psychosocial needs.
    • Non-pharmacological interventions.
    • Documentation of resident mood, behavior, and cognitive changes.
    • Discharge planning documentation.
  • Ensure compliance with dementia care pathways and person-centered practices.
  • Evaluate documentation of:
    • Cultural preferences.
    • Trauma triggers.
    • Behavioral interventions.
    • Effectiveness monitoring of interventions.
  • Participate in survey preparedness and serve as social services representative during surveys.

II. Acute Care, Emergency, Swing Bed
Serve as Licensed Social Worker providing required supervision of Social Services to ensure the following criteria are met:
  • Provide psychosocial assessments for hospitalized and swing bed patients.
  • Coordinate discharge planning and transitions of care.
  • Assist patients with:
    • Advanced directives.
    • Financial assistance.
    • Insurance eligibility.
    • Community resource linkage
  • Provide crisis intervention as needed.
  • Document interventions in the Electronic Health Record (EHR).

III. Home Health Social Services
Serve as Licensed Social Worker providing required supervision of Social Services to ensure the following criteria are met:
  • Provide psychosocial assessment to evaluate a patient's living situation, mental health, and ability to care for themselves to spot non-medical barriers to their recovery.
  • Collaborate with Home Health to ensure continuity of care.
  • Provide resource navigation to connect patients and caregivers to essential services like financial assistance, Meals on Wheels, long-term care planning, and transportation.
  • Assisting patients and their families in coping with stress, grief, and lifestyle changes associated with chronic illness, aging, or terminal diagnoses.
  • Advocacy & Education: Helping patients navigate complex health insurance systems, completing advance directives (e.g., living wills), and ensuring patient rights are protected.
  • Care Coordination: Collaborating with nurses, physical therapists, and physicians to develop personalized discharge and home safety plans.

IV. Rural Health Clinics
  • Provide psychosocial assessment to evaluate a patient's living situation, mental health, and ability to care for themselves to spot non-medical barriers to their recovery.
  • Provide resource navigation to connect patients and caregivers to essential services like financial assistance, Meals on Wheels, long-term care planning, and transportation.

Education

Experience
