Responsibilities
Social Worker - Full-Time - Onsite
Must have experience working in a skilled nursing facility.
The Social Worker provides medical social services to residents in accordance with physician orders and under the direction and
supervision of the community's Social Services Manager. This position assesses residents' social, financial and emotional needs;
and makes appropriate referrals for resolution of problems and facilitates aspects of case management regarding resident transitions through the continuum of care in the community.
Qualifications
Qualified applicants are rqquired to possess 2 years related experience with at least 1 year of experience in a health care setting (hospital, clinic, rehabilitation center, etc.). Bachelor's Degree in Social Work, Psychology, Gerontology or Sociology from an accredited institution is required. Additional requirements include excellent problem solving skills, using a customer-focused approach to problem solving and goal setting, excellent communication and listening skills, enthusiastic, resourceful, organized and possesses strong personal awareness of nonverbal communication. Computer skills including proficiency in MS Office.

Pay Range
USD $59,681.81 - USD $74,596.06 /Yr.
