Job description

JOB SUMMARY:

Under the direction of the Outreach and Enrollment Manager, the Community Outreach Specialist is responsible for the development of relationships with potential referral sources within the PACE service area. This position serves as the liaison between the community and PACE of West Baltimore to generate referrals of appropriate potential enrollees into the program. The Community Outreach Specialist is responsible for educating the public, community partners in care and referral sources about the PACE program, organizing community outreach and promotional activities and guiding interested elders into the intake process. As part of the team, the Community Outreach Specialist identifies and cultivates new relationship opportunities that will support the organization’s goals for enrollment growth and evaluates results. The Community Outreach Specialist serves as the primary sales representative to individuals and organizations who serve the frail elderly. This position also supports and educates community partners and referral sources on other lines of business.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Engages in and coordinates all marketing, outreach and community engagement activities.
  • Identifies opportunities to collaborate with community organizations.
  • Establishes and maintains relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities and potential referral sources in the service area.
  • Develops/participates in speaking engagement opportunities for this position and other members of the PACE team.
  • Provides education to referral sources and the community on PACE services and other services.
  • Participates in the development of the annual strategic marketing/community engagement plan to achieve annual enrollment targets.
  • Exceeds targets for developing contacts/network of referral sources monthly.
  • Engages and partners with the IDT to expand referral source network.
  • Participates in enrollment cross-training initiatives to develop skills in multiple functional areas and support team flexibility.
  • Collaborate with the PACE enrollment team to meet/exceed monthly enrollment targets.
  • Engages in the enrollment process as necessary to support the enrollment team.
  • Assist with development, oversight, implementation and adherence to policies, procedures and standards for marketing and community engagement activities.
  • Assist in the development and administration of the PACE marketing budget in cooperation with the Outreach and Enrollment Manager and Executive Director.
  • Assists in the development of additional marketing brochures and collateral pieces and approval process through CMS.
  • Responsible for adherence to all regulations both federal and state which govern the operations of a PACE organization and a licensed adult day care center.
  • Communicate with participants, families, contracted providers and the public about issues related to the routine operation of the center.
  • Maintain confidentiality regarding participant, staff, contractor and organizational information.
  • Attend and participate in staff meetings, in-services, projects and committees assigned.
  • Adhere to and support the center’s policies, practices and procedures.
  • Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
  • Be flexible in schedule of hours worked.


KNOWLEDGE, SKILLS AND ABILITIES:

  • Engages in and coordinates all marketing, outreach and community engagement activities.
  • Knowledgeable as an advocate for risk-based long-term care programs specifically the PACE model.
  • Exceptional public speaking, communication and customer service skills.
  • Basic familiarity with CRM software.
  • Ability to work effectively and harmoniously with staff, elderly, public and private agencies, governmental officials, providers of services and peers.
  • Energetic, well organized, dependable, flexible and resourceful.
  • Effective oral and written communication skills.
  • Working knowledge of desktop application software (Microsoft Office).
  • Evidence of achieving sales goals within the market and/or demonstrated track record of consistently exceeding corporate goals. Knowledge of healthcare, support services, physician, hospital, skilled nursing facility, case management and discharge planning service needs.
  • Experience working in growth, business development, community partnerships or patient-facing sales in a healthcare context. Prior experience in a PACE organization preferred.
  • Experience working independently including managing own schedule and developing a busy calendar of activities.
  • Ability to travel locally within the program services area.
  • Familiarity with Medicaid enrollment process is preferred.
  • Operate in full compliance with all applicable federal, state and local, health, safety, sanitation, emergency procedures and other standards prescribed in law or regulations.

MINIMUM QUALIFICATIONS:

  • Bachelor’s degree in Marketing, Business or Healthcare related field or relevant experience in lieu of degree.
  • Minimum of 3 years experience in growth, business development, marketing, patient-facing sales, community outreach/partnerships in a healthcare delivery setting or in a managed care environment.
  • Minimum of 1 year experience working with the frail or elderly population.
  • Personal vehicle to travel locally within the program service area.
  • Must submit to and pass a pre-employment criminal background check.
  • Must have a current driver’s license and driving record must be in compliance with company policy; must maintain current automobile insurance.
  • Must have up to date vaccinations according to CMS/ PACE guidelines including vaccination against the seasonal flu and Covid -19.
