This position offers a full-time remote work setting, providing flexibility while ensuring that our specialists can meet the diverse needs of our clients. The standard work schedule is Monday to Friday from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client circumstances. On-call availability may also be required to provide urgent support during off-hours. We emphasize a team-based approach and encourage open communication across disciplines, ensuring an integrated care model.

The Licensed Clinical Social Worker will be responsible for:

• Conducting comprehensive assessments of clients’ psychosocial needs, taking into account their emotional, mental, and social situations.
• Developing personalized and actionable care plans in collaboration with clients and their families to set and achieve specific objectives.
• Offering ongoing counseling, emotional support, and guidance to seniors, helping them navigate transitions such as aging, loss, or health changes.
• Advocating for clients’ rights and ensuring access to community resources necessary for their care.
• Collaborating seamlessly with interdisciplinary teams, including caregivers, health professionals, and other social workers, to enrich the clients’ care experience.
• Conducting regular site visits to evaluate clients' living conditions and well-being, addressing safety issues as they arise.
• Facilitating family meetings to improve communication, resolve conflicts, and work toward unified care goals for the client.
• Educating clients and families on potential community resources, support groups, and programs tailored to enhancing their social engagement.
• Maintaining accurate and timely documentation of assessments, care plans, and progress notes in compliance with ethical standards and regulatory requirements.

This position primarily focuses on seniors, a population that often faces numerous challenges related to aging. As a Social Worker, you will work closely with older adults experiencing health transitions, loss of autonomy, and emotional distress. Understanding these unique needs and building a supportive environment will be critical to your success and client satisfaction.

• Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Clinical Social Worker (LCSW).
• At least two years of experience in a social work role, ideally within community settings or geriatrics.
• Extensive knowledge of social work principles, aging-related issues, and community resources available to seniors.
• Strong assessment, counseling, and problem-solving capabilities.
• Exceptional interpersonal skills, with the ability to build rapport with clients and their families.
• Proficient in electronic health record (EHR) systems and basic computer applications.

While the minimum experience requirement is two years, familiarity with interdisciplinary collaboration alongside healthcare professionals in a community health setting is preferred. Experience working directly with older adults and understanding the complexities of their care is highly valued.

This position offers a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience.
