Job Description:
The Hospice/Outpatient Bereavement Coordinator provides support to patients and family members through the final stages of illness by coordinating hospice services. This role includes offering services in various settings including private homes, skilled nursing facilities, and others, focusing on providing comprehensive bereavement support.
Responsibilities:
  • Coordinate hospice and bereavement services for patients and families.
